Clinical relevance of discography combined with CT scanning. A study of 100 patients

Abstract: Two different classifications of discograms have been used in a prospective study of 279 injected discs in 100 patients. The five-stage classification of Adams, Dolan and Hutton (1986) showed increased degeneration in the lower lumbar discs and more degenerative changes in men than in women. Exact reproduction of the patient's pain on injection was more common in fissured or ruptured discs than in less degenerate discs, with 81% sensitivity and M% specificity of the discogram for pain. The additional informati… Show more

“…In our study concordant pain response was reproduced in 50.8% of the discs. Moreover, Antti-Poika et al have reported a 53% positive rate, which is similar to our finding 9 . Accordingly, we consider that disc degeneration and annular rupture may be two important factors related to discogenic pain.…”

“…It is a subjective test relying on the radiologists' and patients' perceptions to determine the result. [29][30][31][32][33][34][35][36] Many patients with such complaints have associated psychological or psychiatric disturbances which may or may not be associated with medicolegal factors. All of these decrease their ability to give an accurate opinion as to whether the pain produced at discography is that of which they are complaining.…”
