Clostridium difficile Disease in a Department of Surgery

Abstract: A clustering of Clostridium difficile-associated disease in a department of surgery prompted a program of infection control and the evaluation of contributing factors. Fifty patients had diarrhea and positive assays for C difficile cytotoxin during the study period. Twenty-one of the 36 cases that developed among patients admitted to the surgical services occurred on two adjacent general surgery wards that shared attending surgeons and house staff. Perioperative prophylactic antibiotics predated C difficile-as… Show more

“…Risk of CDI can be as high as 8.4% among patients undergoing colorectal, vascular, or cardiothoracic surgery; these patients often receive therapeutic antibiotics in addition to PAP [4][5][6][7][8][9][10][11]. Prior case reports, case series, and small cohort studies restricted their analyses to a single type of intervention.…”

“…Antibiotics associated with low risk of CDI include aminoglycosides, bacitracin, metronidazole, teicoplanin, rifampin, chloramphenicol, tetracyclines, carbapenems, daptomycin and tigecycline [6]. It is worthy of note that in hospitals with a high incidence of CDI, a single dose of antibiotic given in the framework of perioperative prophylaxis may lead to the development of CDI [14]. …”
