Clozapine-induced, dilated cardiomyopathy: a case report

Abstract: BackgroundClozapine is known to be a highly effective antipsychotic agent and additionally provides a significant reduction in suicide-risk and aggression. Clozapine-induced cardiomyopathy is a very rare but nonetheless dangerous side-effect, with an incidence of 0.02–0.1%, depending on the literature, and a mortality rate of up to 17.9%.Case presentationWe report on the case of a 25 year-old patient, who was admitted for the evaluation of a potential electroconvulsive therapy due to persistent auditory halluc… Show more

“…178 The clinical manifestations of clozapine-induced cardiomyopathy range from subclinical forms to severe ones with fulminant pulmonary edema and cardiogenic shock. 179,180 Often the symptoms are insidious and include persistent tachycardia at rest, palpitations, arrhythmias, dyspnea, chest pain, malaise and other signs and symptoms that may be underestimated by the clinicians until the symptoms resemble the clinical picture of heart failure or myocardial infarction. 181 Myocarditis is an uncommon, dose-independent, life-threatening adverse effect and occurs in up to 3% of treated patients.…”

“…41 In other detailed case reports, the use of clozapine as treatment for SP finally led to DCM. 42 , 43 , 44 This implies that SP and DCM may share biological pathways and that the treatment may target the process that is important to both.…”

“…However, one clinical investigation has shown that after neuroleptics of SP, patients had a significantly increased possibility to get DCM 37 . In other detailed case reports, the usage of clozapine as the treatment to SP finally lead to DCM [38][39][40] . This implies that SP and DCM may share biological pathways and the treatment may target at the process that is important to both.…”
