Cluster Headache in Two Sisters. Pupillary Response to Phenylephrine and Tyramine

Abstract: Two sisters with cluster headache were studied with respect to the pupillary responses to instillation into the conjunctival sac of a single drop of a 1% solution of phenylephrine and a 2% solution of tyramine. The changes in pupillary diameters were documented by photographic pupillometry prior to and at 15, 30, 60, and 90 minutes after the instillations.Of the two sisters, one (case A) was examined during a symptom-free interval, when she had been free from cluster headache attacks for 2 1/2 years. When the … Show more

“…Through a general review of the literature, we found and included 7 additional articles that were not identified by our search criteria. Ultimately, 40 articles were identified that met the predetermined eligibility criteria: 22 large cohort studies, 1 twin-based study, and 17 case reports and case series . Large cohort studies were defined as studies that examined at least 20 patients with cluster headache for a possible family history of cluster headache, and case reports or case series were defined as studies with fewer than 10 patients with cluster headache; there were no studies that included between 10 and 20 patients.…”

“…These were suggested to be due to central autonomic dysfunction. Local autonomic function studies in CH, in particular pupillometric studies, suggested lesions of the sympathetic tract (8, 9), although no conclusions were drawn about a possible central or peripheral site of such a lesion.…”

“…For the differential diagnosis, we excluded cluster headache because she had no other autonomic symptom 7 . We also excluded episodic mydriasis because episodic mydriasis is neurologically benign isolated pupillary dilatation.…”
