Coagulation dysfunction in patients with AECOPD and its relation to infection and hypercapnia

Abstract: Chronic obstructive pulmonary disease (COPD) is one of the common chronic airway obstructive diseases, characterized by persistent respiratory symptoms and airflow limitation. Chronic respiratory diseases was recognised as the third leading cause of death in 2017 and COPD was the most common cause of chronic respiratory disease-attributable deaths. 1 It is estimated that COPD will become the fourth largest cause of death in the world and potentially the direct cause of 4.4 million deaths. 2 This will place a s… Show more

“…Many studies [12][13][14][15] suggest that the body is in a long-term hypoxic state, which is easy to activate the coagulation system and promote the simultaneous occurrence of secondary fbrinolytic hyperfunction and hypercoagulability. Patients with AECOPD have long-term airfow restriction and hypoxia, resulting in an increase in the number of secondary red blood cells and a decrease in deformability, resulting in an increase in blood viscosity, promoting platelet adhesion and aggregation, and the formation of microthrombosis [16,17]. Tere is no consensus on the indications for anticoagulation and the duration of anticoagulation in COPD patients.…”

“…Chronic obstructive pulmonary disease (COPD) is a common chronic inflammatory disease of the respiratory system [ 1 ]. The acute exacerbation of COPD (AECOPD) is characterized by incomplete reversible airflow, progressive development, and even respiratory failure due to airway constriction, increased mucus secretion, airflow restriction aggravation, and further ventilation dysfunction [ 2 , 3 ]. The incidence has been on a rise and has become one of the leading causes of death worldwide.…”

“…Moreover, HCT and blood viscosity have been found to be considerably higher in AECOPD patients with respiratory failure than in patients without respiratory failure. 38,39 A database study conducted by Chambellan et al showed a positive correlation between HCT and PaCO 2 and a negative correlation with mortality. 40 The study's findings stated that HCT was a risk factor for HRF, and as the per-single unit of HCT increased, the probability of HRF in AECOPD patients increased by 11.1%.…”
