Cognitive challenges to minimising low value care

Abstract: Clinical decisions often rely on pattern recognition, simple rules of thumb, tacit knowledge and habit. In many instances, such intuitive decisions are fast and accurate, but they can be subject to cognitive biases leading to delivery of care of low value at odds with scientific evidence of best practice. If programmes, such as EVOLVE (Evaluating Evidence, Enhancing Efficiencies) and Choosing Wisely are to have maximal impact in minimising low value care, such biases, and the factors that hide and accentuate t… Show more

“…After the results obtained in this work, regarding the validity of the mobility and activity subscales of the Braden Scale as predictors of pressure ulcer risk, it is reasonable to consider how professionals make decisions using a methodology that adds little value, versus other simpler and more efficient ones, that involved the nurse as an expert on the patient [ 24 , 25 ]. In this sense, a change in practice could be proposed using these subscales due to their high predictive value to identify the risk of pressure ulcers quickly and efficiently [ 1 ].…”

“…Deprescribing inappropriate medications in older people subject to polypharmacy was shown to be safe and effective, and indeed reduced mortality . Cognitive biases that may predispose to clinical decisions that result in low value care were introduced to clinical audiences in an article, which attracted considerable professional and media interest, and was circulated within college circles …”
