Collagen Injections for Urinary Stress Incontinence in a Small Urban Urology Practice: Time to Failure Analysis of 99 Cases

Abstract: Collagen injection has minimal morbidity and a low median success rate. Bladder instability, as determined by cystometrogram, is an important determinant of success. Survival statistics methods should be used more widely to describe urological outcomes because they provide dynamic and, thus, more meaningful information to urologists and patients than summary statistics.

“…These 19 women were then compared to the 92 women within the cohort, who were successful (dry) for over 1 month, but did not achieve a durable response to collagen following their treatment. These 92 women had a mean of 2.25 (1)(2)(3)(4)(5)(6)(7)(8) injections and remained dry according to stress tests for a mean period of 247 days (28-1,305). In this analysis (Table 2), prior surgery showed a trend toward a successful long-term response (p=0.07).…”

“…Dry rates after collagen injections have ranged from 23 to 96% in most reports, with a fall off in efficacy over time [3][4][5][6][7][8][9][10]. The definition of incontinence "cure" for collagen injections tend to represent improvement, which has been reported to last an average of 7 months.…”

“…However, 6 months is theoretically an adequate timeframe to evaluate outcomes of urethral injections because studies show a median duration of success for Contigen injections of 4.7 months [2]. Generally, Contigen degrades over 6 to 9 months, often necessitating repeat injections.…”

“…Studies have shown quality-of-life improvements up to 24 months after Contigen injections for the treatment of ISD [1]. However, the procedure is hampered by poor long-term durability and the frequent need for readministration, as the Contigen degrades over time [2]. In response to these concerns, carbon-coated zirconium beads suspended in β-glucan gel (Durasphere, Carbon Medical Technologies, Saint Paul, MN, USA) have been used because they are nonabsorbable.…”
