Colonic adenocarcinoma associated with the acquired immune deficiency syndrome

Abstract: Colonic adenocarcinoma developed in an intravenous drug abuser with the acquired immune deficiency syndrome (AIDS) that was diagnosed by the presence of antibodies to the human immunodeficiency virus (HIV), generalized lymphadenopathy, and biopsy proven esophageal candidiasis. The colon cancer presented atypically at a young age with no known risk factors and with a bulky primary tumor and a local fistula. AIDS and AIDS risk factors have been associated with Kaposi's sarcoma, lymphomas, and anal and oropharyng… Show more

“…11 Furthermore, a few studies suggest that colorectal cancers may present with more aggressive behavior with HIV. [12][13][14] Thus, it is possible that the risk of cancer is elevated with an AIDS-defining opportunistic intestinal infection, due to the infection's associated sustained inflammatory response. Therefore, we analyzed the impact of AIDS-defining intestinal infections on risk of colon cancer using the HIV/AIDS Cancer Match Study data.…”

“…Several authors suggest the presence of aggressive tumor behavior and young age in patients with gastrointestinal cancers associated with HIV infection [3,6,9,10]. Read et al [10] described a 26-year-old homosexual man with AIDS who presented with Listeria monocytogenes sepsis and small-cell carcinoma of the rectum.…”

“…Cappell et al [6] reported a 36-year-old man, an intravenous drug abuser, who presented with fatigue and weight loss. He had a large mass in the sigmoid colon, occupying the entire pelvis, involving the urinary bladder, and producing bilateral hydronephrosis.…”

“…While some studies have failed to find a link between HIV and gastrointestinal (GI) cancer, several epidemiological studies have demonstrated an elevated risk and earlier age of onset of colonic neoplasia in the HIV-positive population, [1][2][3] including a recent study from the United States that reported a significant increase in colorectal cancer during the period from 2000 to 2003. 4 Further evidence supporting increased gastrointestinal cancer in the context of HIV comes from studies of colorectal precursors and lesions where the increased prevalence of adenomas and advanced neoplasia was recognized when comparing the HIV-positive versus HIVnegative population.…”
