Colorectal cancer screening in Lynch syndrome: Indication, techniques and future perspectives

Abstract: Lynch syndrome (LS) is an inherited predisposition to colorectal cancer (CRC), responsible for 3-5% of all CRC. This syndrome is characterized by the early occurrence of colorectal neoplastic lesions, with variable incidences depending on the type of pathogenic variants in MMR genes (MLH1, MSH2, MSH6, PMS2 and EPCAM) and demographics factors such as gender, body mass index, tobacco use and physical activity. Similar to sporadic cancers, colorectal screening by colonoscopy is efficient because it is associated … Show more

“…The two conditions often alternate, patients may develop intestinal obstruction in the late stage, and some patients will experience anemia and weight loss. Thus, the physical quality of patients has deteriorated sharply, and the difficulty of treatment has increased [5][6][7]. CRC patients are also prone to tumor cell metastasis, and lymphatic metastasis is relatively common out of tumor metastasis.…”

“…Patients have lost the best opportunity for surgery, and surgical resection can no longer achieve good results. In clinical practice, targeted therapy or chemotherapy is usually the main method [ 5 , 6 ]. Epidermal growth factor receptor (EGFR) is overexpressed in many tumor tissues and participates in the proliferation, differentiation, and apoptosis of tumor cells [ 7 , 8 ].…”

“…With the progression of the disease, symptoms such as abdominal pain, anemia, abdominal mass and hematochezia will gradually appear. Most patients have progressed to the intermediate and advanced stage when they are diagnosed, and the cancer cells metastasize to a distance, so their lives can only be prolonged by chemotherapy ( 16 , 17 ). At present, the oxaliplatin-based combination regimen of XELOX and FOLFOX is mainly used for patients with advanced colorectal cancer who miss the opportunity of surgery.…”
