Colorectal cancer screening in the COVID-19 era

Abstract: Colorectal cancer (CRC) is the third most diagnosed form of cancer and second most deadly cancer worldwide. Introduction of better screening has improved both incidence and mortality. However, as the coronavirus disease 2019 (COVID-19) pandemic began, healthcare resources were shunted away from cancer screening services resulting in a sharp decrease in CRC screening and a backlog of patients awaiting screening tests. This may have significant effects on CRC cancer mortality, as delayed screening may lead to ad… Show more

“…Our study suggests that while these decisions can be extremely challenging, they are important and necessary considerations to being fast, adapting, calculating and ethically mindful. A prioritisation model for CRC screening and colonoscopy follow-up was essential during this time and has also been alluded to by others 18 35 44 50 51. The need for this type of prioritisation protocol is likely to extend to other crises and may also be useful in addressing the lingering backlog in screening and diagnosis resulting from the pandemic 43 52.…”

“…In order to mitigate the survival decline attributed to the interruption of the CRC screening pathway elicited by the COVID-19 pandemic, public health screening programs need to be restructured [40]. Loveday et al noted that prioritizing the high-risk CRC patients via FIT triage would alleviate 89% of deaths that would occur due to the lockdown backlog [37].…”

“…Note that throughout the pandemic, the use of telemedicine picked up to meet the needs of the healthcare system. This approach extends to CRC screening, where telehealth appointments regarding CRC screening have increased [39,40]. Telehealth visits are still ongoing, even as in-person CRC screenings in hospitals or other medical settings are starting to resume [41][42][43].…”
