Colorectal Neoplasm in Asymptomatic Average-risk Koreans: The KASID Prospective Multicenter Colonoscopy Survey

Abstract: The overall prevalence of colorectal neoplasm in asymptomatic average-risk Koreans is comparable with that in Western countries. Being male and older are associated with a higher risk of colorectal neoplasm. Over half of proximal neoplasms are not associated with any distal sentinel lesions.

“…Our estimates of the prevalence of colorectal adenomas are consistent with previous studies based on more selected and smaller screening samples (7,11). In one study, the prevalence of colorectal adenomas among average risk asymptomatic Koreans 50 years of age or older were 39.9% and 23.0% among men and women, respectively (7), whereas in another study, the prevalence among Korean men 40 to 69 years of age was 36.3% (11).…”

“…These participants had no symptoms of colorectal disease, no personal history of colorectal cancer, polyps, or inflammatory bowel disease, no colon examination within the last 10 years, no previous colonic surgery, and no family history of colorectal polyps. Similarly, a multicenter colonoscopy survey conducted in 2003 to 2004 among 11 university hospitals in Korea found prevalence rates of 33.3% and 2.2% for colorectal adenomas and advanced adenomas, respectively (7). Although it is assumed that the incidence of colorectal adenomas is increasing in proportion to colorectal cancer rates, there is limited data on recent adenoma trends in Korea (8).…”

“…The incidence of colonic adenoma has increased rapidly in Korea. In a recent multicenter study, the prevalence rate of colonic neoplasm in asymptomatic Korean adults was 33.2%, which was similar in Western countries (2,3). However, patients with removed adenoma remain at high risk of developing new adenomas or cancer in the subsequent years, which justified follow-up colonoscopy (4).…”

“…The purpose of the screening program was to promote health through early detection of chronic diseases and their risk factors. Such programs are popular in Korea (10). In addition, in Korea, the Industrial Safety and Health Law requires employees to participate in annual or biennial health examinations.…”
