Colostomy for Perianal Sepsis With Ecthyma Gangrenosum in Immunocompromised Children

Abstract: BACKGROUND Perianal sepsis with ecthyma gangrenosum is a severe and potentially mutilating complication in immunocompromised children. Therapies include antimicrobial treatment, incision and drainage, generous tissue debridement, and skin transplantation. PROCEDURE We describe 3 children with acute lymphoblastic leukemia having sepsis with Pseudomonas aeruginosa in febrile neutropenia and severe perianal infections treated relatively early with a protective colostomy. Indications for colostomy were nonhealing … Show more

“…Due to the high of mortality of the condition, reported as high as 54% in perineal lesions associated with bacteremia, 5 aggressive medical treatment with combination antibiotic therapy is warranted, 6 , 7 and multiple surgical procedures, including extensive surgical debridement and diverting colostomy, are needed. 2 , 4 , 8 …”

“…Surgical treatment (incision and drainage) has been shown to improve the survival of patients with a perirectal abscess in a small case series [11]. Surgical approaches vary from a simple incision and drainage to a defunctioning colostomy [3,13,15,16,19,20,21]. However, surgical intervention can be limited by many factors, such as frailty, a bleeding tendency, and a high risk of a subsequent infection in leukemia patients.…”

“…Contemporary construction of an ostomy is often temporary with an intent to protect a downstream anastomosis by keeping the area clean from stool passage [102,103]. The goal of creating a diverting ostomy is hence to reduce rates of clinically apparent colorectal anastomotic leakage and to decrease severe complications and reoperation rates [103][104][105][106]. However, diverting ostomies are associated with considerable morbidity, patient inconvenience, and hospital cost [107].…”
