Combination Therapy of Ipilimumab and Nivolumab-associated Toxic Epidermal Necrolysis (TEN) in a Patient With Metastatic Melanoma: A Case Report and Literature Review

Abstract: Ipilimumab and nivolumab are immune checkpoint inhibitors used in the treatment of metastatic melanoma. The authors report the case of a 62-year-old white male individual with metastatic choroidal melanoma who had commenced adjuvant systemic treatment with combination checkpoint inhibitor therapy of intravenous ipilimumab (anti-cytotoxic T-lymphocyte antigen-4) and nivolumab (anti-programmed cell death-1) at 3-week cycle intervals. On day 4 after the second cycle, he developed an acute widespread rash. On exam… Show more

“…39 In 1.2%-4% of patients, the rash is considered severe (grade 3) 4,38 and it may be the initial presentation of a potentially life-threatening skin reaction. [40][41][42] Histopathologic features include a superficial, perivascular lymphocytic infiltrate predominantly with eosinophils and CD4+ T cells, with or without epidermal spongiosis and papillary dermal edema. 5 The treatment is largely symptomatic.…”

“…[4][5][6]38,46 Rarely, life-threatening conditions such as SJS, TEN, and DRESS may develop. [40][41][42] In such cases, treatment with immunotherapy should be immediately and permanently discontinued.…”

“…There has been an increasing number of case reports describing more uncommon and unusual adverse events, e.g., lipodystrophy [24]. Severe cutaneous adverse events, such as toxic epidermal necrolysis, are rare [25]. Other disorders that may be triggered by immune checkpoint blockade include plaque psoriasis and lichen planus (occasionally with mucosal involvement) (Figure 2) [21].…”
