Combined Surgical and Medical Therapy for Candida Prosthetic Endocarditis in a Patient with Repaired Tetralogy of Fallot

Abstract: Candida prosthetic endocarditis (CPE) is an uncommon and fatal complication in adults with congenital heart disease. The current guidelines for the management of fungal endocarditis recommend a combination of surgical and medical therapy. However, it still remains uncertain when surgical management in CPE patients should be performed. Therefore, the prognosis of CPE patients is very poor. Here we report a case of CPE in a 31-year-old woman who had undergone surgical repair for tetralogy of Fallot during childh… Show more

“…Among them, we excluded 714 reports for the following reasons: combinations other than antifungals (n = 438), reviews (n = 138), off-topics (n = 103), languages other than English (n = 19), no fungal identification (n = 12) and unspecified combination therapy (n = 4). “…Early diagnosis and prompt treatment are crucial for preventing disease progression and death. 1,2) Here we report a case of IE in a patient with tetralogy of Fallot (TOF) undergoing surgery and transcatheter pulmonary valve implantation (TPVI). She was successfully managed with a course of antibiotic therapy without significant pulmonary valve regurgitation.…”

“…1 Although the majority of these cases are caused by Staphylococcus and Streptococcus species, less common organisms, such as Candida, have been reported. 2 Infective endocarditis can present with atypical symptoms, including some cases of idiopathic/immune thrombocytopenic purpura (ITP) caused by atypical infections such as Mycobacteria. 3 Fungal IE accounts for less than 2% of total cases, though has up to 47% in-hospital mortality.…”
