Combined use of stent-retriever and aspiration thrombectomy for cerebral venous sinus thrombosis involving the straight sinus: A case report

Abstract: Cerebral venous sinus thrombosis (CVST) is generally treated with anticoagulation therapy, however, endovascular therapy is considered for refractory cases. A 42-year-old woman presented with a progressive disturbance in consciousness, and diagnosed CVST in the straight sinus (SS). Recanalization of SS was achieved by emergent thrombectomy using aspiration catheter and stent-retriever system, and her level of consciousness improved immediately. The combined use of them aided the removal of the thrombu… Show more

“…But there has been difficulty in tracking these devices up into the sinuses due to steep angles at various junctions [ 16 ]. In such instances, a stent retriever or a balloon can play the role of an anchor to assist in navigating these devices into the sinuses [ 16 - 18 ]. But the aspiration catheters used in these cases were older generation reperfusion catheters.…”

“…A combined technique with a stent retriever and an aspiration catheter (stent anchor with mobile aspiration technique) for CVST was first reported by Raychev et al in 2014. 10) There have been some reports treated with the combined technique captured thrombus by both the stent retriever and the aspiration catheter 7,11) as in the present case, and it is considered an effective treatment method. In addition, there have been some reports of the combined use of aspiration thrombectomy and intrasinus thrombolysis using a penumbra aspiration catheter.…”

“…A stent retriever may also be used as an anchor while the distal access (aspiration) catheter is passed back and forth to aspirate the thrombus 89. Alternatively, a stent retriever and thrombus are removed into the aspiration catheter or as a unit into the large proximal guide sheath under ongoing continuous aspiration 90. This allows the thrombus to be captured by both aspiration and stent retriever devices.…”
