Adult subjects with classical phenylketonuria (PKU) who were diagnosed and treated neonatally participated in this long-term follow-up study. Twenty-four subjects received neuropsychological (NP) assessment and a subset received magnetic resonance imaging (MRI) and magnetic resonance spectroscopy (MRS) to identify: (1) pattern of cognitive dysfunction; (2) effect of high blood phenylalanine (Phe) level at time of cognitive testing; and (3) treatment variables that may be associated with cognitive difficulties in adulthood. All subjects had average IQ except one subject in the borderline range. Diet was initiated by the 15th day of life. All subjects except one were on diet until age 6 years (mean years of treatment = 15). Blood Phe levels at cognitive testing ranged from 157 to 1713 micromol/L (mean = 1038); 11 subjects had levels < 1000 micromol/L and 13 subjects had levels >1000 micromol/L. Results suggest that adults with early-treated PKU demonstrate specific cognitive deficits, a number of which are associated with the frontal and temporal area of the brain. Deficits were noted in several domains including executive functioning, attention, verbal memory, expressive naming and verbal fluency. Self-report measures of depression and anxiety were generally in the normal/mild range. The group with a Phe level > 1000 micromol/L scored lower than the group with Phe level < 1000 micromol/L on measures of focused attention, verbal fluency, reaction time, verbal recognition memory, visual memory and naming. Tests of cognitive functioning were often correlated with measures of treatment during childhood rather than with Phe level at the time of cognitive testing. Subjects with abnormal MRI scored significantly lower on two cognitive tests (Trails A and CVLT Recognition Memory). We found no significant correlation between current brain Phe level obtained through MRS (n = 10) and neuropsychological functioning. Future longitudinal investigation with a larger sample size will assist in clarifying the aetiology of neuropsychological deficits and association with treatment history.

La fenilcetonuria (FCU) es un error metabólico congénito que implica una deficiencia de la enzima fenilalanina hidroxilasa. Este proceso da lugar a niveles elevados de fenilalanina y niveles bajos de tirosina. Si no se administra tratamiento aparecen graves secuelas neuropatológicas y neuroconductuales. La puesta en práctica de la detección sistemática de recién nacidos y el tratamiento dietético precoz ha reducido significativamente esta morbilidad. A pesar de una función intelectual general relativamente conservada, en los pacientes con FCU tratados precozmente se detectan todavía sutiles deficiencias cognitivas y conductuales. Este trabajo aporta una revisión de los datos del deterioro de la elaboración de información, la función ejecutiva, la memoria, el aprendizaje, el aprovechamiento escolar y la conducta. En este trabajo también revisamos las dos teorías principales propuestas hasta la fecha en relación con los mecanismos subyacentes de las deficiencias cognitivas y conductuales halladas en pacientes con FCU tratados, concretamente la disfunción prefrontal y las anomalías de la sustancia blanca. Aunque es necesario ampliar la investigación, la literatura disponible hasta la fecha da a entender que los pacientes afectados de FCU tratados precozmente presentan un riesgo de deficiencias neuroconductuales insidiosas en toda una gama de dominios neuropsicológicos. Por lo tanto, los pacientes con FCU tienen que ser monitorizados minuciosamente para detectar precozmente estos problemas y aplicar intervenciones apropiadas capaces de asegurar que estos pacientes alcancen su pleno potencial.

La phénylcétonurie (PCU) est une erreur innée du métabolisme causée par un déficit de l’activité de la phénylalanine hydroxylase, induisant une phénylalaninémie élevée et une tyrosinémie faible. Si cette maladie n’est pas traitée, elle engendre une neuropathologie sévère associée à des troubles neurocomportementaux. Le dépistage néonatal et la mise en place précoce d’un régime alimentaire réduisent significativement ces morbidités. Malgré un fonctionnement intellectuel général relativement bien préservé, un déficit cognitif léger et de faibles troubles comportementaux persistent chez les patients traités. Cet article présente les données sur les altérations du traitement de l’information, des fonctions exécutives, de la mémoire et l’apprentissage, de la réussite scolaire et du comportement. Il présente également les deux premières théories, le dysfonctionnement préfrontal et les anomalies de la substance blanche, proposées pour expliquer les mécanismes sous-jacents de ces déficits cognitifs et comportementaux observés chez les patients traités. Bien que des recherches supplémentaires soient nécessaires, la littérature actuelle suggère que les patients phénylcétonuriques traités précocement pourraient présenter des risques de troubles neurocomportementaux légers dans différents domaines neuropsychologiques. Ces patients doivent donc être examinés régulièrement de façon à pouvoir détecter ces troubles le plus tôt possible et ainsi pouvoir mettre en place des interventions adaptées permettant à ces patients d’atteindre leur potentiel optimal.
