Commercial Cannabinoid Oil-Induced Stevens-Johnson Syndrome

Abstract: Purpose. To report an unusual presentation of commercial cannabidiol (CBD) oil-induced Stevens-Johnson Syndrome/toxic epidermal necrolysis (SJS-TEN). Methods. A 56-year-old woman presented with acute onset of a diffuse, blistering, maculopapular rash with over 30% total body surface area (BSA) involvement two days after taking CBD oil sublingually for chronic pain. Biopsy confirmed SJS-TEN. Ophthalmology was consulted and mild eye involvement was found. She was started on topical cyclosporine, prednisone, moxi… Show more

“…9 Most recently, SJS was reported in a woman two days after receiving sublingual CBD oil. 10 As with the dog in our report, the woman developed widespread ulceration including oral and palpebral involvement. The correlation between hemp oil exposure and the CADR was considered "probable" based on the Naranjo Adverse Drug Reaction Probability Scale, 11 given the following attributes: lesion appearance after administration, resolution after discontinuation of the oil, reports suggestive of drug-induced cutaneous lesions from cannabinoid products in people, and lack of support for alternative causes other than exposure to the CBD oil-containing product (Table S2).…”

“…Although cannabidiol is the active ingredient in the implicated product, other cannabinoids or terpenoids cannot be discounted as possible contributors. Coconut oil is listed as a carrier in this product, and has been implicated in a case of SJS in a woman, 10 but causation seems unlikely in this dog given the low concentration of coconut oil.…”

“…9,10 Data on the prevalence and types of adverse effects of topical cannabis are lacking. A recent case of fatal Stevens-Johnsons Syndrome was reported upon the use of CBD spray sublingually, 26 reminding users and prescribers that serious adverse events are possible with products often thought of and marketed as natural.…”

“…The literature regarding side effects of CBD was considerably expanded since the writing of our article, so that besides the anecdotal reports and paediatric studies already mentioned, several case reports 8 – 10 , a survey 9 , a meta-analysis of clinical trials 11 and a post marketing safety assessment of a full spectrum hemp extract 12 became available. Specifically the survey 9 reports observations of side effects including “feeling high”, an effect likely to be explained by Δ 9 -THC contamination rather than by CBD.…”
