Aims and objectives:The aim of the present study was to determine the effect of different intracanal medicaments on the apical seal of BioRoot root canal sealer (RCS). Materials and methods: One hundred permanent single-rooted teeth were used in this study. All the samples were decoronated at the cementoenamel junction and instrumented in a sequential order from 15 to 50 number k-file. The specimens were randomly divided into five groups containing 20 teeth each. Intracanal medicaments used in this study were Metapex, triple antibiotic paste, Metrogyl DG gel forte (metronidazole gel 1.5% w/w), and curenext gel. For control group following instrumentation, the roots were obturated with laterally compacted gutta-percha with BioRoot RCS. In medicament groups after the period of 14 days, the medicaments were removed. All the four groups were obturated with BioRoot RCS and gutta-percha cones using lateral compaction technique. All the specimens were coated with nail varnish and immersed in 2% methylene blue. Then the specimens were demineralized and diphanized. The cleared teeth were analyzed by means of a stereomicroscope under 10× magnification. All the data were analyzed in SPSS version 18 software (IBM, Chicago, IL, USA). Result: Among all the intracanal medicaments, triple antibiotic paste showed the highest microleakage. When Metpaex and Metrogyl DG gel forte were compared with Curcuma longa, differences were not statistically significant. Conclusion: Among all the intracanal medicaments, triple antibiotic paste showed the highest microleakage and least was Metapex. Clinical significance: Incomplete removal of medicaments prevents the penetration of sealer into the dentinal tubules and interferes with the normal setting reaction, thus affecting the seal of obturating material leading to microleakage and subsequent treatment failure. Hence, while placing an intracanal medicament, it is important to consider its effect on leakage of the root canal system.

Aim: The main aim of the present study is in vitro assessment of the diffusion ability and degradation of calcium hydroxide from pastes prepared by using different vehicles to determine the influence of a vehicle on pH of calcium hydroxide [Ca(OH) 2 ] pastes. Materials and methods: A total of 44 single-rooted premolar teeth were used in this study. The tooth crowns were removed and the root canals were prepared. Depending on the vehicle to be used for preparing calcium hydroxide pastes, four groups were made: group I: Ca(OH) 2 saline paste (control group), group II: Ca(OH) 2 neem , group III: Ca(OH) 2 aloe vera , group IV: Ca(OH) 2 turmeric paste. After biomechanical preparation, calcium hydroxide herbal paste dressings were applied and sealed with resin-based cement. The teeth were placed in containers with distilled water, and the pH of the water was measured at regular intervals over 3, 24, and 168 hours. Results: The present study showed all herbal pastes allowed the diffusion of ions, but pastes prepared with aloe vera showed more ion diffusion and a marked increase in pH, depicting better support for calcium hydroxide action. Conclusion: Aloe vera allows better diffusion of calcium hydroxide through dentinal tubules, thus enhancing its action, and advises its use as a vehicle for placing intracanal medicament. Clinical significance: Calcium hydroxide [Ca(OH) 2 ] is the most common and effective intracanal medicament. However, CH is not so effective when used alone. Therefore combinations are required to enhance its clinical properties. To be effective, diffusion of hydroxyl ions are required for which vehicle plays a significant role.

Aim:To compare the antimicrobial efficacy of herbal irrigants neem, miswak, propolis with sodium hypochlorite using conventional needle irrigation and EndoVac irrigation system against Enterococcus faecalis. Materials and methods:A total of 120 extracted single-rooted mandibular premolar teeth were infected for 21 days with E. faecalis after instrumentation with ProTaper system. Before irrigation procedure, dentinal shavings were collected in 1 mL of sterile broth and incubated. The optical density of each broth was measured using digital colorimeter and initial readings were recorded. Samples were then divided into four groups of 30 teeth each -Group I: Sodium hypochlorite irrigation, group II: Neem irrigation, group III: Miswak irrigation, group IV: propolis irrigation. Each group was further divided into two subgroups -(a) EndoVac irrigation (b) conventional needle irrigation. After irrigation, dentinal shavings were collected and optical density recorded. The values were analyzed statistically with Student's t test and analysis of variance followed by Tukey's honest significant difference test; p-value < 0.05 was considered to be statistically significant. Results:The postirrigation optical densities in all the groups were significantly lower than preirrigation values. Sodium hypochlorite demonstrated better antimicrobial efficacy followed by propolis, neem, and miswak. Differences in optical density values for all irrigants are higher in EndoVac (p < 0.0001) compared with needle (p = 0.0009) group, but it failed to reach statistical significance. Conclusion:Sodium hypochlorite proved to be a better irrigant followed by propolis, neem, and miswak. EndoVac irrigation system was more effective for elimination of E. faecalis than needle irrigation group.
