Comparison of Autologous Serum Eye Drops with Different Diluents

Abstract: In the eyes with Sjögren syndrome and persistent epithelial defects, AS(100) was the most effective in decreasing symptoms, corneal epitheliopathy and promoting fast closure of wound. In the eyes with non-Sjögren syndrome, AS(100) and AS(50NS) have similar effects in decreasing symptoms and corneal epitheliopathy.

“…In addition, administration of artificial tears, viscoelastic (such as hyaluronan or chondroitin sulfate) eyedrops, punctal plugs (Tai et al, 2002), and fibrin glue (Sumich et al, 2003) as well as improved lid hygiene have been tried. Autologous serum eyedrops (Geerling et al, 2004;Matsumoto et al, 2004;Mukerji et al, 2002;Noble et al, 2004), umbilical cord serum (Vajpayee et al, 2003;Yoon et al, 2005Yoon et al, , 2007, EGF (Daniele et al, 1992), and plasma rich in growth factors (Cho et al, 2013;Geerling et al, 2004;Kim et al, 2012;Lopez-Plandolit et al, 2010;Mukerji et al, 2002;Poon et al, 2001;Tsubota et al, 1999b;Vajpayee et al, 2003) have also been examined in the clinical setting, but none of these treatment modalities has been confirmed to be effective scientifically (Pan et al, 2013).…”

“…Numerous studies 19,20,[22][23][24][25][26][27][28]45 involving humans with persistent corneal epithelial defects have revealed beneficial effects of topically applied serum on epithelial healing, but epithelial deficiency may not be a primary contributor to the pathogenesis of SCCED in dogs. Although abnormal histologic features have been demonstrated in the epithelium of up to 94% of dogs with SCCED, 6 similar findings have also been reported for dogs with chronic, experimentally induced corneal wounds.…”

“…In a recent study comparing different concentrations, 50 % AS with sodium hyaluronate was more effective than artificial tears in reducing symptoms, corneal epitheliopathy and promoting fast closure of corneal wounds in non-Sjögren patients, while in Sjögren patients 100 % serum was the more effective choice. 46,47 Exactly the optimal concentration of AS eye drops is not clearly known in the treatment of severe DED. An important component of AS eye drops which makes the dilution of AS to 20 % necessary is transforming growth factor beta (TGF-β).…”

“…That is one of the reasons why 20 % dilution was used in a whole of the previous AS studies in the literature. 46,47 Punctal plugs may be used with caution in severe DED. Silicone or collagen plugs could be used initially for a trial period, and then a permanent punctal occlusion with cautery should be considered.…”
