ImmunoCyt/uCyt+™ improves the sensitivity of urine cytology in patients followed for urothelial carcinoma

ImmunoCyt/uCytt is a fluorescent test combining three monoclonal antibodies. In this study, it has been tested as a complement to cytology in the detection of urothelial carcinoma in urine. It has been performed simultaneously with standard cytology and cystoscopy on 870 urine analyses from one hospital. In 136 cases, one or more bladder tumors were found. Overall sensitivity of cytology, ImmunoCyt/uCytt and combined analyses reached 29, 74 and 84%, respectively, and overall specificity was 98, 62 and 61%. The negative predictive value of cytology, ImmunoCyt/uCytt and both analyses was 88, 93 and 95%, respectively, and the positive predictive value was 70, 26 and 29%. The sensitivity of cytology for low malignant potential neoplasms, low-and high-grade papillary carcinomas was 6, 18 and 53%, while it reached 71, 79 and 93% when combined with ImmunoCyt/uCytt. The sensitivity of cytology for stages Ta, T1, T2 and over and Tis tumors was12, 67, 47 and 50%, while it reached 78, 83, 79 and 100% when combined with ImmunoCyt/uCytt. In the absence of tumor on cystoscopy but with positive ImmunoCyt/uCytt, 18% of patients developed a tumor, 2-6 months later. Of the 109 cases diagnosed as suspicious for malignancy by cytology, a tumor was present in 30 cases and ImmunoCyt/uCytt was positive in 22 (73%) of them. In conclusion, ImmunoCyt/uCytt may be used to postpone cystoscopies in patients followed for bladder cancer and may help to save cytologist and pathologist screening Keywords: urothelial carcinoma; cytology; ImmunoCyt/uCytt; immunofluorescence; urinary bladder; recurrence Superficial bladder cancers are defined as tumors limited to the mucosa (Tis, Ta) or invading into the lamina propria (T1), without muscle invasion. 1 They represent more than 80% of urothelial carcinomas and more than 50% will recur. 1-3 Patients with bladder cancer are followed with regular cytologies and cystoscopies. Cytology is very specific but this test is limited by its low sensitivity ranging from 16 to 60%. 4 Cytology is most useful at detecting highgrade (HG) cancer, whereas its sensitivity for lowgrade (LG) urothelial tumors is low and merely reaches 17%. 4 Furthermore, criteria used in urinary cytology to detect tumor cells are largely subjective and the ability to detect cancer cells is dependent on the experience of cytologists or pathologists. For instance, atypical cells and papillary aggregates may be found in either reactive or neoplastic conditions. 5 Furthermore, LG urothelial carcinomas do not always exfoliate in aggregates and the cell characteristics are often so subtle that they might not be recognizable even by an experienced cytologist. 5 Standard practice in the follow-up of patients with bladder cancer requires cystoscopies at regular intervals, but this technique is invasive, costly and causes discomfort to the patient. RESUMEN PRUEBAS DE MARCADORES TUMORALES EN EL CÁNCER DE VEJIGAEl patrón de referencia para detectar un cáncer de vejiga es la cistoscopia, que puede llegar a identificar casi todas las lesiones papilares y sésiles. Es, sin embargo, un procedimiento invasivo que causa molestias a los pacientes. La citología urinaria es el marcador no invasivo estándar con una especificidad muy alta, pero con baja sensibilidad negativa para los tumores de vejiga Ta, G1 y T1. Para mejorar la detección precoz del cáncer de vejiga y para monitorizar la respuesta al tratamiento y la recurrencia tumoral, la opción serían los marcadores tumorales vesicales. Una prueba ideal para el cáncer de vejiga debería tener la posibilidad de sustituir o retrasar la cistoscopia en el seguimiento de los pacientes con cáncer de vejiga. En los últimos años, la FDA aprobó las pruebas de marcadores tumorales no invasivas ImmunoCyt / uCyt+, BTA TRAK, BTA stat, NMP22, NMP22 BladderChek y UroVysion que hemos investigado. Las pruebas demostraron mayor sensibilidad para el diagnóstico del cáncer de vejiga en comparación con la citología urinaria. En conjunto, la sensibilidad media y la especificidad media fue del 64-80% y del 71-95%, y los valores predictivos positivo y negativo medios para detectar los procesos malignos fueron del 49-84% y del 79-95%, respectivamente. Los ensayos BTA TRAK, BTA stat, NMP22 y NMP22 BladderChek están limitados por el número de resultados falsos positivos en pacientes con enfermedades urológicas benignas, por ejemplo hematuria, urocistitis, cálculos renales o infecciones de las vías urinarias. Debido a su baja especificidad BTA TRAK, BTA stat, NMP22 y NMP22 BladderChek no deben usarse sin descartar primero la presencia de una enfermedad genitourinaria benigna o maligna aparte del cáncer de vejiga. A excepción de UroVysion que alcanza un 80% de sensibilidad y un 94% de especificidad, ninguna de estas pruebas no invasivas ha puesto de manifiesto una sensibilidad y especificidad elevadas al mismo tiempo, que es la principal demanda que debe hacerse a un marcador tumoral ideal. Una sensibilidad insuficiente junto con una especificidad limitada no permiten sustituir la cistoscopia en el diagnóstico del cáncer de vejiga ni tomar decisiones terapéuticas basándose en un resultado de prueba positivo.Palabras clave: Cáncer de vejiga. RESUMENESTUDIO COMPARATIVO ENTRE CISTOSCOPIA, CITOLOGÍA URINARIA, NMP-22 Y UN NUEVO MÉTODO, BLADDER CHEK, EN EL SEGUIMIENTO DEL CÁNCER VESICAL SUPERFICIAL Objetivo: Evaluar la utilidad de la determinación cualitativa del NMP-22 en el seguimiento del carcinoma vesical superficial en pacientes asintomáticos, comparándolo con su determinación cuantitativa, la citología y la cistoscopia.Material y Métodos: 88 pacientes asintomáticos en seguimiento por carcinoma vesical superficial se les recogió una muestra de orina recién emitida, repartiéndose en 3 alícuotas, para citología, NMP-22 y 4 gotas se aña-den al dispositivo Bladder chek, para su lectura en 30 minutos. Posteriormente, cistoscopia y RTU-vesical en sospecha de recidiva.Resultados: 26 pacientes tenían recidiva tumoral y 62 estaban libres de enfermedad. La sensibilidad fue del 28% para el Bladder chek, 34.62% para el NMP-22, 34.62% para la citología y de 100% para la cistoscopia. La especificidad fue de 93.55%, 80.33%, 87.10% y 87.10% respectivamente. La sensibilidad por grado fue: 25 en G1, 28.57 en G2 y 50 para G3 en Bladder chek, de 29.41, 42.86, 50 para NMP-22 respectivamente, 23.53, 71.43, 0 para la citología. Por estadios la sensibilidad fue: 27.27 para Ta-1 y 50 en T2 para Bladder chek, 34.78, 50 para NMP-22 y 39.13, 0 para la citología.Conclusiones: La baja sensibilidad del Bladder chek lo invalida como método alternativo a la cistoscopia en el seguimiento del carcinoma vesical superficial asintomático. Materials and Methods: A simple of urine just voided was taken in 88 asymptomatic patient follow-up for superficial bladder cell carcinoma. This dose was distributed in 3 parts, for performed cytology, for determination of NMP-22, and 4 drops of the third part are added to device bladder chek. Later, we performered cystoscopy and transurethral resection in patients with a suspicion of bladder cancer.Results: 26 patients had tumor relapse and 62 patients were free of disease. The sensitivity for the bladder chek was of 28%, 34.62% for NMP-22, 34.62% for cytology and 100% for cystoscopy. The specificity was of 93.55%, 80.33%, 87.10% and 87.10% respectively. The sensitivity by degree was 25 in G1, 28.57 in G2 and 50 in G3 for Bladder chek; 29.41, 42.86and 50 for NMP-22; 23.53, 71.43 and 0 for cytology. The sensitivity by stages was 27.7 in Ta-1 and 50 in T2 for Bladder chek; 34.78 and 50 for NMP-22; 39.13 and 0 for the cytology.Conclusions: The low sensitivity of bladder chek invalidates it like alternative method to the cystoscopy in the follow-up of the superficial asymptomatic bladder cell carcinoma.
