Introduction:Orthopedic Trauma exerts a holistic influence on survivors' physical health including a range of mental health problems which interfere with survivors' recovery. Psychiatric disorders and behavioral disturbances are reported to be 3-5 times more common among people with injuries and are predictor of poor outcome and ongoing disability. Therefore, assessing depression and anxiety among orthopedic trauma patients play a vital role in implementing further interventions. Methods:Institutional based cross-sectional study was conducted at Tikur Anbessa specialized hospital from May 29-June 30, 2017. Hospital anxiety and depression scale was used to assess anxiety and depression by using face to face interview. Systematic sampling technique was used to select a total of 407 participants. Data has been analyzed using SPSS 20. Bivariate and multivariate logistic regression was done to identify associated factors. Variables with p-value <0.05 have been considered as statistically significant.Result: Prevalence of depression and anxiety were 36.1% and 39.8% respectively. Being female (AOR=2.3595%CI (1.48,3.72)), poor social support (AOR=2.5195%CI (1.30,4.85)), developing complication (AOR=1.9195%CI (1.07,3.52)), presence of amputation (AOR=3 .6495%CI (1.60,8.24)) and having pain (AOR=2.0295%CI (1.24,3.30)) for depression and being female (AOR=1.9995%CI(1.11,3.57)), having chronic medical illness (AOR=3.0795%CI(1.36,6.92)), having family history of mental illness (AOR=2.24 95%CI (1.05,5.4.91)), lower extremity injury (AOR=2.93 95%CI (1.38,6.21)) and having severe pain (AOR=2.75 95%CI (1.32,5.74)) for anxiety had significant association at p-value <0.05.Conclusion: prevalence of depression and anxiety were high. Being female, having poor social support, developing complication, presence of amputation and having pain for depression; and being female, having chronic medical illness, having family history of mental illness, lower extremity injury and having severe pain for anxiety were significantly associated factors. It is good if clinicians give emphasis for orthopedic patients especially for females and with chronic medical illness.

IMPORTANCENo consensus has been reached to date regarding the optimal treatment for distal radius fractures. The international rate of operative treatment has been increasing, despite higher costs and limited functional outcome evidence to support this shift. OBJECTIVESTo compare functional, clinical, and radiologic outcomes after operative vs nonoperative treatment of distal radius fractures in adults. DATA SOURCES The PubMed/MEDLINE, Embase, CENTRAL (Cochrane Central Register of Controlled Trials), and CINAHL (Cumulative Index to Nursing and Allied Health Literature) databases were searched from inception to June 15, 2019, for studies comparing operative vs nonoperative treatment of distal radius fractures. STUDY SELECTION Randomized clinical trials (RCTs) and observational studies reporting on the following: acute distal radius fracture with operative treatment (internal or external fixation) vs nonoperative treatment (cast immobilization, splinting, or bracing); patients 18 years or older; and functional outcome. Studies in a language other than English or reporting treatment for refracture were excluded. DATA EXTRACTION AND SYNTHESIS Data extraction was performed independently by 2 reviewers. Effect estimates were pooled using random-effects models and presented as risk ratios (RRs) or mean differences (MDs) with 95% CIs. Data were analyzed in September 2019. MAIN OUTCOMES AND MEASURES The primary outcome measures included medium-term functional outcome measured with the Disabilities of the Arm, Shoulder and Hand questionnaire (DASH) and the overall complication rate after operative and nonoperative treatment.RESULTS A total of 23 unique studies were included, consisting of 8 RCTs and 15 observational studies, that described 2254 unique patients. Among the studies that presented sex data, 1769 patients were women [80.6%]. Overall weighted mean age was 67 [range, 22-90] years). The RCTs included 656 patients (29.1%); observational studies, 1598 patients (70.9%). The overall pooled effect estimates the showed a significant improvement in medium-term (Յ1 year) DASH score after operative treatment compared with nonoperative treatment (MD,]; P = .005; I 2 = 84%). No difference in complication rate was observed (RR, 1.03 [95% CI, 0.69-1.55]; P = .87; I 2 = 62%). A significant improvement in grip strength was noted after operative treatment, measured in kilograms (MD, 2.73 [95% CI,]; P = .04; I 2 = 79%) and as a percentage of the unaffected side (MD, 8.21 [95% CI,]; P = .007; I 2 = 76%). The goal of this prospective study was to evaluate the real-life experience of 52 patients and their recovery kinetics in the first 6 weeks after surgical treatment of distal radius fractures. The fractures were treated with either a fourth-generation volar locking plate (34 patients, mean age 67 years, range 54-82) or by percutaneous pinning (18 patients, mean age 56 years, range 43-69). These patients were evaluated every week for 6 weeks with the QuickDASH (QD) questionnaire. A lower QD score indicated that the patient's physical function and symptoms had improved. At postoperative week 1, all the patients who had undergone percutaneous pinning had a QD of 80 (out of 100). One-third of patients who had undergone plate fixation had a QD of 80, while the remainder had a QD of 65. Out of all the patients who had a QD of 80 at postoperative week 1, the patients who had undergone plate fixation improved more quickly. After 6 weeks, the patients who had undergone plate fixation had a greater reduction in the QD (50 points) than the ones who had undergone pinning (30 points). Although the direct costs of locking plate fixation are 10 times higher than those of K-wire pinning, evidence suggests that these plates reduce the indirect costs. This study has shown that a patient's recovery rate, day-to-day life and physical function are better over the first 6 weeks postsurgery when the distal radius fracture is fixed with a locking plate.
