Comparison of Diagnosis-Specific Survival Scores for Patients with Small-Cell Lung Cancer Irradiated for Brain Metastases

Abstract: Diagnosis-specific survival scores including a new score developed in 157 patients with brain metastases from small-cell lung cancer (SCLC) receiving whole-brain radiotherapy (WBRT) with 30 Gy in 10 fractions (WBRT-30-SCLC) were compared. Three prognostic groups were designed based on the 6-month survival probabilities of significant or almost significant factors, (age, performance score, number of brain metastases, extra-cerebral metastasis). Six-month survival rates were 6% (6–11 points), 44% (12–14 points) … Show more

“…In the present study, the MST was 11.87 months, which is relatively better than in previous reports (14,20). This was mainly due to the retrospective design and an inherent selection bias.…”

“…Patients were then grouped into three prognostic stratifications according to their total points (18). However, this system is complex and has limited predictive ability, and is thus not often used in clinical practice (19,20). In 2018, Bernhardt et al proposed a novel index called the BMS-Score, which consists of only three factors: KPS, synchronous BM or metachronous BM, and ECM state (15).…”

“…In the second part of the present study, the new scoring tool was compared to three existing diagnosis-specific instruments designed from patients with cerebral metastases from SCLC of any age (7)(8)(9). The four tools were compared regarding the PPVs to correctly identify patients dying within 6 months and patients surviving for at least 6 months.…”

“…In addition, the new tool was compared to existing diagnosis-specific instruments created for patients with cerebral metastases from SCLC of any age, namely the updated diagnosis-specific graded prognostic assessment classification (DS-GPA), the Rades-SCLC score and the WBRT-30-SCLC score (7)(8)(9). Comparisons were performed with respect to the positive predictive values (PPVs) to correctly identify patients who die within 6 months following WBI (comparison of the least favorable groups) and patients who survive for at least 6 months following WBI (comparison of the most favorable groups).…”
