Objective This review aimed to systematically determine the optimal nasal saline regimen for different types of sinonasal diseases. Data Sources PubMed, Embase, SCOPUS, Cochrane Library, Web of Science, ClinicalTrials.gov. The last search was on December 6, 2021. Review Methods Study selection was done by 2 independent authors. Randomized controlled trials and meta-analyses were included. The effects of nasal saline treatment through various devices, saline tonicities, and buffer statuses were evaluated in patients with allergic and nonallergic rhinitis, acute and chronic rhinosinusitis (CRS), CRS with cystic fibrosis, and postoperative care, including septoplasty/turbinoplasty and endoscopic sinus surgery. Results Sixty-nine studies were included: 10 meta-analyses and 59 randomized controlled trials. For allergic rhinitis, large-volume devices (≥60 mL) were effective for treating adults, while low-volume devices (5-59 mL) were effective for children. Isotonic saline was preferred over hypertonic saline due to fewer adverse events. For acute rhinosinusitis, saline irrigation was beneficial in children, but it was an option for adults. Large-volume devices were more effective, especially in the common cold subgroup. For CRS, large-volume devices were effective for adults, but saline drop was the only regimen that had available data in children. Buffered isotonic saline was more tolerable than nonbuffered or hypertonic saline. The data for CRS with cystic fibrosis and nonallergic rhinitis were limited. For postoperative care, buffered isotonic saline delivered by large-volume devices was effective. Conclusion Nasal saline treatment is recommended for treating most sinonasal diseases. Optimal delivery methods for each condition should be considered to achieve therapeutic effects of saline treatment.

ABSTRAK Latar Belakang: Rinosinusitis kronis (RSK) adalah kelainan peradangan pada sinus paranasal yang paling umum menyebabkan gejala sinonasal kronis. Rinosinusitis kronis (RSK) merupakan salah satu kondisi medis kronis yang paling umum terjadi di seluruh dunia, memengaruhi semua kelompok usia. Angka kejadiannya diperkirakan sekitar 12,3% di Amerika Serikat, 10,9% di Eropa, dan 13% di China. RSK juga merupakan kondisi yang menyebabkan penurunan signifikan dalam kualitas hidup pasien. Ahli Telinga, Hidung, dan Tenggorokan (THT) baru-baru ini menemukan bahwa penggunaan larutan garam hipertonik untuk irigasi hidung lebih efektif daripada menggunakan larutan garam isotonik untuk tujuan yang sama. Larutan garam hipertonik, yang memiliki tekanan osmotik lebih tinggi, mampu mengurangi edema mukosa dengan tingkat efikasi yang lebih besar. Tujuan: Tujuan dari penelitian ini adalah untuk menyelidiki efikasi irigasi hidung dengan larutan garam hipertonik pada rinosinusitis kronis: tinjauan sistematis dan meta analisis. Tinjauan pustaka: Dengan mengkaji kriteria Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, penelitian ini memastikan mematuhi semua persyaratan terkini. Strategi pencarian mencakup beberapa basis data referensi elektronik (Pubmed dan SagePub) dan mencakup publikasi yang diterbitkan antara tahun 2015 dan 2023. Kami mengekslusi publikasi duplikat, tinjauan artikel , dan artikel yang tidak lengkap. Meta-analisis digunakan untuk menganalisis hasil skor VAS. Hasil: Hasil pencarian kami di basis data PubMed menghasilkan 35 artikel, sementara di SagePub kami menemukan 28 artikel. Hasil pencarian untuk tahun terakhir 2015 menghasilkan 18 artikel untuk PubMed dan 12 artikel untuk SagePub. Pada akhirnya, kami memiliki total 8 artikel (5 dari PubMed dan 3 dari SagePub), pada akhirnya kami mendapatkan empat studi yang memenuhi syarat. Dari setiap studi individu yang kami masukkan dalam tinjauan sistematis, kami menemukan bahwa larutan garam hipertonik lebih baik dalam mengurangi gejala pasien. Namun, hanya dua studi yang cocok untuk dilakukan meta-analisis. Kami tidak menemukan perbedaan signifikan dalam skor VAS antara pasien yang menerima larutan garam hipertonik dan larutan garam normal (selisih rata-rata 2,40 [95% CI -2,85 – 7,65]; p 0,37). Kesimpulan: Irigasi hidung dengan larutan garam hipertonik untuk rinosinusitis kronis menunjukkan efek samping sedang dan meningkatkan perbaikan gejala hidung serta aktivitas silia lebih baik daripada larutan garam isotonik, meskipun tidak ada perbedaan signifikan dalam pencitraan radiologi atau penciuman.
