Objective: Delaminated rotator cuff tears are a common shoulder disorder in elderly individuals. Either arthroscopic separate double-layer repair (DR) or en masse repair (ER) is used to treat a delaminated rotator cuff tear. We conducted this systematic review and meta-analysis to compare the clinical outcomes of arthroscopic ER versus DR intervention. Methods: Five studies were acquired from PubMed, Medline, Embase, CNKI, Google, and the Cochrane Library. The data were extracted by two of the coauthors independently and were analyzed with RevMan 5.3. Mean differences (MDs), odds ratios (ORs), and 95% confidence intervals (CIs) were calculated. The Cochrane Collaboration's risk of bias tool and Newcastle-Ottawa Scale were used to assess the risk of bias. Results: Five studies, including two randomized controlled trials (RCTs) and three observational studies, were assessed. The methodological quality of the trials ranged from low to high. The pooled results for the Shoulder Rating Scale of the University of California at Los Angeles (UCLA) score, visual analog scale (VAS) score, Constant score, and range of motion (ROM) showed that the outcomes were not statistically significant between the two interventions. The difference in retear rate was not statistically significant (OR = 0.69, 95% CI = 0.36-1.33, P = 0.27). The sensitivity analysis proved the stability of the pooled results, and publication bias was not apparent. Conclusions: Both arthroscopic ER and DR interventions had benefits in delaminated rotator cuff tear treatment. ER and DR treatments were equally effective and had the same retear rate. The arthroscopic DR technique could not be recommended as the optical choice for delaminated rotator cuff tears based on current evidence.

PurposeThis study aimed to clarify the characteristics of delaminated rotator cuff tears (RCTs) and evaluate the clinical outcomes of a modified arthroscopic en masse suture bridge repair for delaminated RCTs.MethodsPatients with full‐thickness RCTs, who underwent arthroscopic suture bridge repair with a minimum 2‐year follow‐up, were retrospectively reviewed. Patients were categorized into two groups based on the presence of delamination. Delaminated RCTs were treated using a modified en masse suture bridge technique, while nondelaminated RCTs received a conventional suture bridge technique. Preoperative and postoperative Constant scores and American Shoulder and Elbow Surgeons (ASES) scores were determined to evaluate clinical outcomes. Postoperative magnetic resonance imaging (MRI) was carried out to identify the integrity and retear of the repaired rotator cuff.ResultsA total of 172 patients were included in our study cohort, in which 67 (39%) delaminated RCTs were confirmed intraoperatively. The prevalence of delamination was significantly higher in large tears (53/102, 52%) compared to medium tears (14/70, 20%) (p < 0.001). No significant differences in age (n.s.) or gender (n.s.) were observed between the two groups. Both groups showed significant improvements in Constant and ASES scores postoperatively (both p < 0.001), with no significant differences between the groups (n.s.). The retear rates were 2/67 (3.0%) in the delamination group and 3/105 (2.9%) in the nondelamination group, showing no significant difference (n.s.).ConclusionsThe modified arthroscopic en masse suture bridge technique was effective for repairing delaminated RCTs, yielding favourable clinical outcomes comparable to those of nondelaminated tears.Level of EvidenceLevel IV.

Delamination of rotator cuff tears presents a challenge to shoulder arthroscopists. Tear pattern recognition and an understanding of anatomy as it relates to the superior capsule guide treatment strategies. The key to management of a delaminated rotator cuff tear is to recognize differential retraction of respective layers. Successful outcomes of surgical management require conscientious and deliberate restoration of the attachment points of the cable. The goal is to produce an anatomic repair.
