Purpose To compare the results of silicone and polypropylene Ahmed glaucoma valves (AGV) implanted during the first 10 years of life. Methods A prospective study was performed on 50 eyes of 33 patients with paediatric glaucoma. Eyes were matched to either polypropylene or silicone AGV. In eyes with bilateral glaucoma, one eye was implanted with polypropylene and the other eye was implanted with silicone AGV. Results Fifty eyes of 33 children were reviewed. Twenty five eyes received a polypropylene valve, and 25 eyes received a silicone valve. Eyes implanted with silicone valves achieved a significantly lower intraocular pressure (IOP) compared with the polypropylene group at 6 months, 1 year, and 2 years postoperatively. The average survival time was significantly longer (P ¼ 0.001 by the log-rank test) for the silicone group than for the polypropylene group and the cumulative probability of survival by the log-rank test at the end of the second year was 80% (SE: 8.0, 95% confidence interval (CI): 64-96%) in the silicone group and 56% (SE: 9.8, 95% CI: 40-90%) in the polypropylene group. The difference in the number of postoperative interventions and complications between both groups was statistically insignificant. Conclusion Silicone AGVs can achieve better IOP control, and longer survival with less antiglaucoma drops compared with polypropylene valves in children younger than 10 years.

A szerző az előrehaladott glaucomás károsodás esetén hatékony műtéti eljárásokat tekinti át. Magas nyomású, előre-haladott glaucoma esetén alacsony szemnyomás elérése szükséges. Ezt az első műtétként, megfelelő pre-és posztoperatív gyulladásgátló kezelés mellett végzett, mitomycin C-alkalmazással kombinált trabeculectomia biztosítja a legeredményesebben. Sok esetben azonban a glaucomaellenes fi ltrációs műtét sikertelenné válik a fokozott episcleralis hegesedés miatt. Ilyenkor a hosszú elvezetőcsövű csarnokvíz-elvezető implantátum beültetése a választandó megoldás. Az Ahmed implantátummal Magyarországon több mint egy évtizedes kedvező tapasztalattal rendelkezünk a legkomplikáltabb, magas szemnyomású, előrehaladott glaucomás esetek megoldásában. Az előrehaladott glaucoma korszerű műtéti ellátásának lényegét a nem szemész orvosnak is célszerű ismernie, hiszen a kezelésre vonatkozó tanácsaival segítheti glaucomás betegei optimális ellátását. In high pressure advanced glaucoma a low target intraocular pressure is to be set. The most effective method of reaching this target pressure level is mitomycin C trabeculectomy combined with effective pre-and postoperative antiinfl ammatory treatment. However, in several cases glaucoma fi ltering surgery gradually fails due to increased episcleral fi brosis. In these cases use of long-tube glaucoma drainage devices is recommended. In Hungary use of the Ahmed implant has provided favourable clinical outcome even in the most complicated, high pressure, advanced glaucoma cases. Modern types of surgical treatment of advanced glaucoma need to be known by non-ophthalmologist physicians for more than a decade. This may help them to propose the optimal treatment modality to their glaucoma patients. A 40 évesnél idő-sebb kaukázusi népességben a glaucoma prevalenciája legalább 2%. A betegség prevalenciája az életkor előre-haladásával fokozódik, a nyolcadik életévtizedre a betegek aránya 7% körülire nő [2]. A glaucoma népegészség-ügyi jelentősége éppen ezért világszerte és hazánkban is kiemelkedően nagy [1,3]. A glaucoma nem egyenlő az emelkedett intraocularis nyomással. A retinalis ganglionsejtek kórosan felgyorsult ütemű apoptózisát az emelkedett szembelnyomás mellett a látóidegfő (papilla nervi optici) visszatérő fokális ischaemia-reperfúzió káro-sodása (alacsony, illetve ingadozóan alacsony ocularis perfúziós nyomás) és genetikai eltérések is kiváltják. Introduction: Glaucoma valves have been used in the treatment of glaucoma in the cases where medications no longer show the appropriate results. Due to the various complications in molteno, other glaucoma valves such as Ahmed glaucoma valve (AGV) were introduced. Ahmed glaucoma valve has proved to be very useful in the management of glaucoma. This review thus summarizes how far AGV has been successful in the management of glaucoma. Methods:Various case studies were taken into consideration in this review. The studies were retrospective amongst which some were randomized and others non-randomized. Patients with different kind of glaucoma such as Aphakic and Neovascular were selected and implanted with AGV. The results of the success rate were then compiled and summarized.
