Comparison of the POP-Q examination, transvaginal ultrasound, and direct anatomic measurement of cervical length

Abstract: The POP-Q examination estimate of CL correlates fairly with aCL at the time of hysterectomy; uCL does not appear to correlate with aCL or eCL. Cervical elongation may be defined as an anatomic length of 5.0 cm or a POP-Q estimate of 8.0 cm.

“…POP-Q examination correlated well with anatomic measurements (r = 0.3, p = 0.005), whereas transvaginal ultrasound examination was less correlated with anatomical measurements (r = 0.19, p = 0.14). 15 The correlation between cervical length examination on the apical component of POP-Q examination and MRI examination found a good correlation (r = 0.41-0.59, p <0.0001). 5 Until now there have been no studies that examine sensitivity, specifi city and accuracy POP-Q examination in measuring cervical length to diagnose cervical elongation in pelvic organ 53 Priyatini,Fernando,Widyakusuma,et al Indones J Obstet Gynecol…”

“…Of note, cervical elongation, defined as lengthening of the cervix independent of prolapse, can confound the diagnosis of apical prolapse. There exists no current consensus definition of cervical elongation, however various studies describe the length of an elongated cervix as ranging from 5 to 8 cm on POP‐Q measurement between points C and D (Finamore et al, ; Dancz et al, ).…”

“…Although ultrasound has been reported to have good reliability in measuring the cervical length in the obstetric literature, it is used to predict the risk of preterm birth [4,5]. Dancz et al [6] measured the cervical length in patients with POP using ultrasonography and found a poor correlation with the gross specimen cervical length; additionally, the interobserver reliability of the ultrasonography measurements was poor compared with that of specimen measurements and ECL. This may be because the lower uterine segment of a non-pregnant woman is not well formed and ultrasonography cannot show a clear boundary with the internal os.…”
