Background: The aim was to compare the diagnostic efficacy of ultrasonography (USG) and hysteroscopy in detecting uterine abnormalities in abnormal uterine bleeding (AUB) by correlating the results with histopathological examination.Methods: This prospective study was conducted among women attending gynecological OPD of Subharti medical college, Meerut over a period of two years from October 2019 to August 2021. A total of 100 perimenopasual women with AUB attending obstetrics and gynaecology OPD were included in this study. All patients underwent transvaginal scan to note down the endometrial thickness and to rule out uterine and adnexal pathology. All the patients underwent diagnostic hysteroscopy, followed by a biopsy of the endometrium using a curette. The endometrium was sent to the pathologist. Findings of these diagnostic modalities then correlated.Results: Out of 100 women, USG detected that 54 patients (54%) had no pathology and 46 patients (46%) had abnormal findings, out of which maximum patients, 29 patients (63.04%) had endometrial hyperplasia. According to hysteroscopy, 46 patients (46%) had normal hysteroscopic findings while 54 patients (54%) had abnormal findings of which maximum were 18 patients (33.33%) who had endometrial hyperplasia. Histopathology findings revealed that 47 patients (47%) had normal findings and 53 patients (53%) had abnormal findings out of which maximum patients 20 (37.7%) had endometrial hyperplasia. In our study of 100 women with AUB, on USG only 1 patient had endometrial malignancy and the same was reported by hysteroscopy and histopathology.Conclusions: In our study hysteroscopy proved to be highly sensitive and specific considering histopathology as gold standard. Ultrasonography has good sensitivity and specificity but less as compared to hysteroscopy.

Background The systematic evaluation of the diagnostic value of hysteroscopy and transvaginal ultrasonography for endometrial hyperplasia. Methods A systematic search was performed using China Knowledge Network Database, Wanfang Chinese Database, China Biomedical Literature Database, China Science and Technology Journal Database, PubMed, Medline, The Cochrane Library, and web of science from their dates of inception to December 31, 2017, hysteroscopy and transvaginal ultrasonography were used for the diagnosis of endometrial lesions in those studies. Two researchers screened the literature, extracted data, and evaluated the methodological quality of the included studies independently based on the inclusion criteria. Results A total of 1,354 cases were incorporate into analysis from 9 diagnostic studies meta-analysis showed that the diagnostic accuracy of hysteroscopy for endometrial hyperplasia lesions were: combined sensitivity =0.73 (95% CI: 0.68–0.77), combined specificity =0.92 (95% CI: 0.90–0.93), combined positive likelihood ratio =9.87 (95% CI: 4.08–23.84), combined negative likelihood ratio =0.34 (95% CI: 0.22–0.52), combined diagnostic odds ratio =31.64 (95% CI: 10.34–96.78), area under the summary receiver operating characteristic curve =0.8830. The combined sensitivity of transvaginal ultrasonography for those lesions were 0.69 (95% CI: 0.64–0.74), combined specificity was 0.78 (95% CI: 0.76–0.81), combined negative likelihood ratio was 3.40 (95% CI: 2.46–4.70), combined negative likelihood ratio was 0.41 (95% CI: 0.34–0.50), combined diagnostic odds ratio was 8.94 (95% CI: 5.80–13.79), and area under the summary receiver operating characteristic curve was 0.8180. Conclusions Compared with transvaginal ultrasonography, hysteroscopy have higher sensitivity, specificity, and area under the summary receiver operating characteristic curve for the diagnose of endometrial hyperplasia, hysteroscopy should be selected priority as the auxiliary diagnosis.

Information about an abundant uterine bleeding and classification of the PALM-COEIN in non-pregnant women of reproductive age are presented. Classification of the PALM-COEIN includes nine major categories of uterine bleeding depending on the etiologic factor. The use of combined oral contraceptive containing estradiol valerate and dienogest is warranted in the COEIN category. The use of combined oral contraceptive with estradiol valerate and dienogest can reduce the amount of menstrual blood loss and the intensity of pain during menstruation. Role of this oral contraceptive is effective to the prevention of recurrent abnormal uterine bleeding and after polypectomy.
