Complex intervention modelling should capture the dynamics of adaptation

Abstract: BackgroundComplexity has been linked to health interventions in two ways: first as a property of the intervention, and secondly as a property of the system into which the intervention is implemented. The former recognizes that interventions may consist of multiple components that act both independently and interdependently, making it difficult to identify the components or combinations of components (and their contexts) that are important mechanisms of change. The latter recognizes that interventions are imple… Show more

“…An insufficiently developed programme or conceptual weaknesses may endanger the implementation in practice, and subsequently affect its effectiveness (Bleijenberg et al 2018;Chalmers and Glasziou 2009). We are in agreement with Greenwood-Lee et al (2016) in suggesting that developing a programme without trying it out in practice seems to be problematic. The outcomes of our study illustrate that developing a`state-of-art´family caregiver support concept in theory alone was not sufficient (Figure 2, preliminary concept).…”

“…Public health intervention researchers seek increasingly to move beyond the simple question ‘what works?’ and, instead, ask questions about why interventions work, how and over which time‐periods effects emerge and decay, who they work for, in which settings and the range of outcomes affected, including alertness to any unanticipated and unintended consequences .…”

“…The pathways through which immunization systems interact with their host health systems are just starting to be examined using methods that allow users to account for complexity. Complexity is a technical term in health systems (and public health) research literature that indicates the importance of relationships and adaptive interactions of components in the emergence of the whole (system) (Greenwood-Lee et al, 2016). Despite representing only one area of service delivery in a health system, the immunization system is an example of complexity.…”
