Composite keratohyaline granules in palmoplantar keratoderma: An ultrastructural study

Abstract: We report the results of an ultrastructural study of the hyperkeratotic epidermis in 23 cases of palmoplantar keratoderma (PPK) comprising 8 inherited keratoderma, 8 keratoderma climactericum in menopausal women and 7 symptomatic keratoderma. In all but one of the cases of inherited PPK and keratoderma climactericum, composite keratohyalin (KH) granules were found in granular cells of the interductal epidermis, which were similar to those found in the rat and in some other conditions. In the cases of symptomat… Show more

“…The occurrence of heterogeneous keratohyaline granules, similar the ones reported by Jessen, have repeatedly been observed in distinct human tissues such as the acrosyringeal cells (Anton-Lamprecht, 1980), the cells of the fetal nail (Breathnach, 1971), the interfollicular epidermis in warts (Lau rent et al, 1978), in certain palmoplantar hyperkeratoses (Laurent et al, 1985), and in the buccal mucosa of multiple fibroepithelial hyperplasia (Haneke, 1982). Moreover, Kastl and Anton-Lamprecht (1990) reported a distinct "bicomponent" keratohyaline in normal human ridged skin.…”

“…The medusa head-like granule has been reported as an unusual type of keratohyalin granule in such pathological cells as epidermal granular cells of actinomycin-D treated rats (9), of viral warts (10), and of palmoplantar keratoderma (11). Occurrence of this unique granule, however, has not been reported in squamous cell carcinoma.…”

“…13 The real significance of the epidermolysis, however, is still uncertain and does not occur exclusively in this variant, but also in other keratodermas, as well as other congenital and acquired diseases (Table 6). 4 Electron microscopy reveals 5,14 17 A citoqueratina K9, cujo peso molecular é de 54Kda, e o ponto isoelétrico (pHi), de 5,4, é encontrada exclusivamente nas superfícies palmo-plantares. 18 Tal característica é a principal responsável por permitir a individualização da queratodermia de Vörner como entidade única, diferenciando-a não só das demais queratodermias, mas também de outras doenças com padrão histopatológico semelhante.…”
