— The epidemiological crisis of 2020–2021 has revealed a number of imbalances and “bottlenecks” that have developed in the Russian healthcare system over the past 20 years as a result of a policy of limiting development to breakthroughs in individual areas accompanied by optimization of the sector. It became evident that one of the most acute problems is interregional disparity in terms of personnel and resource availability in the healthcare system, which determines the system’s ability to respond to challenges and shocks. Solving these problems requires a comprehensive approach: simply increasing the sector’s financing is not sufficient and must be accompanied by structural changes, in particular, modifying the education system and training new highly qualified personnel, creating an effective system of territorial distribution of personnel, and radically increasing the availability of high-end equipment, i.e., a transition to a new model of healthcare.

BACKGROUND: Modern strategic planning for the development of the health care system in the region involves regular monitoring of problem areas for the timely identification of emerging problems, as well as monitoring the effectiveness of measures designed to eliminate the identified problems. Based on this, within the framework of the 5th level of medical and sociological monitoring of the results of strategic planning for the development of regional health care, which involves studying the opinions of regional experts, a study was conducted focused on studying the main problems and prospects for the development of the health care system in the Moscow Region on the most important ones from the standpoint of its strategic planning directions. AIM: To determine the degree of relevance of problems for the health care system of the Moscow Region and for the next medium-term strategic planning of measures for its development, as well as to evaluate the indicators from the standpoint of their significance, which will serve as the basis for compiling the archetype of the health care system in the territory of the municipal formation of the Moscow Region in order to adopt further management decisions, including taking into account municipal features. MATERIALS AND METHODS: A survey was conducted of experts from among the heads of regional medical organizations of the Moscow Region, their deputies for the medical profile of activity, as well as the main freelance specialists of the Ministry of Health of the Moscow Region. A total of 126 people were interviewed using formalized structured interviews and questionnaires. RESULTS: In the course of the study, a range of the most pressing problems of the health care system of the Moscow Region was determined today. In addition, activities were identified, the strategic planning of which is most relevant for the formation of the next medium-term program “Health of the Moscow Region”. The study also made it possible to streamline the list of indicators characterizing the archetype of the health care system in the territory of the municipality of the Moscow Region, according to the degree of their importance in making and implementing management decisions in the framework of medium-term strategic planning for health care development. CONCLUSION: The study demonstrated the sustainability of some problems in the health care system of the Moscow Region that currently need to be addressed.

Guaranteed healthcare financing and population health capital indicators in Russia

BACKGROUND: Financial support for medical care is one of the conditions for the implementation of the state’s social objectives and strategic provision of the required level of health capital for the population. Financial resources of compulsory health insurance account for more than 48% in the structure of sources of financing of guaranteed medical care. AIM: Analysis of the relationship between basic integral indicators of population health capital and the level of financial security of medical care within the framework of the compulsory health insurance program. MATERIALS AND METHODS: The study was carried out using the method of correlation and regression analysis, the method of constructing time series, comparative analysis of quantitative and qualitative indicators. RESULTS: Financial support for medical care within the framework of compulsory medical insurance in 2011-2022. was characterized by significant underfunding and a decrease in the coverage of the population with timely and high-quality medical care. Government expenditures within the framework of compulsory health insurance are determined by the dynamics of GDP per capita. The volume of expenses for the provision of guaranteed medical care does not directly determine the level of life expectancy, birth rate and disability of the population. The scale of financial support within the framework of compulsory health insurance only partially mediates the variation in such indicators of health capital as natural population growth and the level of general morbidity. CONCLUSION: The absence of a formalized positive correlation between indicators of the volume of state financial resources within the framework of compulsory medical insurance and indicators of health capital for 2011-2022, revealed in the framework of the study. can be considered as a direction for increasing the efficiency of the organizational and economic mechanism for using limited resources.
