Objective: To compare the performance of magnetic resonance imaging (MRI) and transvaginal ultrasound (TVU) in detecting deep infiltrating endometriosis (DIE), using Enzian classification. Secondarily, to evaluate the influence of nodule size on the accuracy of MRI and TVU. Methods: A retrospective study was carried out at "La Paz" University Hospital, Madrid, Spain, between April 2012 and December 2014. Inclusion criteria were suspicion of DIE at gynecologic examination, indication to undergo TVU, MRI and surgery. Exclusion criteria were previous hysterectomy, bowel resection, or urinary tract surgery. The diagnosis of DIE using MRI or TVU was considered positive when it correlated with histology.Sensitivity, specificity, accuracy, and mean size of the nodule were calculated. Results:In the present study involving 48 women, TVU demonstrated greater accuracy than MRI for recto-vaginal (77% vs 69%) and vaginal (94% vs 89%) endometriosis. MRI showed greater accuracy (96%) than TVU (92%) for bladder endometriosis. The size of the nodule did not have a significantly different effect on the accuracy of TVU compared with MRI. Conclusion:In the present study, TVU provided a more accurate localization of vaginal and recto-vaginal endometriosis as compared with MRI; however, MRI should be recommended if a suspicion of bladder endometriosis exists. The nodule size did not seem to influence the accuracy of the two techniques.

Diagnostic value of transvaginal three-dimensional ultrasound combined with color Doppler ultrasound for early cesarean scar pregnancy

Background: Our study sought to obtain data which assess the diagnostic value of transvaginal threedimensional ultrasound (3D-US) combined with color Doppler ultrasound (US) for early cesarean scar pregnancy (CSP).Methods: All participants were randomly divided into a Control group diagnosed using 3D-US and a Combination group diagnosed using 3D-US combined with color Doppler US. The preoperative US results were compared with postoperative pathological results. The diagnostic coincidence rate, sensitivity, and specificity of these two examination methods were compared, and their diagnostic results for different types of CSP were analyzed. Finally, the diagnostic effects of both methods were compared and analyzed, and the imaging of CSP was summarized. Results:The diagnostic accuracy of transvaginal 3D-US combined with color Doppler US (92.96%) was significantly higher than that of transvaginal 3D-US (71.83%). For different types of CSP, the diagnostic rate of CSP with mixed echogenic mass and partial implantation of gestational sac in the Combination group was markedly higher than that in the Control group.Conclusions: Additionally, the sensitivity, specificity, and area under the receiver operating characteristic (ROC) curve (AUC) in the Combination group were higher than those in the Control group. Transvaginal 3D-US combined with color Doppler US can improve the sensitivity, specificity, and accuracy of diagnosis of early CSP, and has important reference value for clinical condition evaluation and treatment options.

(1) Objectives: In patients with symptoms suggestive of rectosigmoid endometriosis, imaging techniques are required to confirm the presence and establish the extent of the disease. The objective of the current study was to compare the performance of three-dimensional rectal water contrast transvaginal ultrasonography (3D-RWC-TVS) and computed tomographic colonography (CTC) in predicting the presence and characteristics of rectosigmoid endometriosis. (2) Methods: This prospective study included patients with suspicion of rectosigmoid endometriosis who underwent both 3D-RWC-TVS and CTC and subsequently were surgically treated. The findings of imaging techniques were compared with surgical and histological results. (3) Results: Out of 68 women included in the study, 37 (48.9; 95% C.I. 38.2–59.7%) had rectosigmoid nodules and underwent bowel surgery. There was no significant difference in the accuracy of 3D-RWC-TVS and CTC in diagnosing the presence of rectosigmoid endometriotic nodules (p = 0.118), although CTC was more precise in diagnosing endometriosis located in the sigmoid (p = 0.016). 3D-RWC-TVS and CTC had similar precision in estimating the largest diameter of the main endometriotic nodule (p = 0.099) and, in patients undergoing segmental resection, the degree of the stenosis of the bowel lumen (p = 0.293). CTC was more accurate in estimating the distance between the lower margin of the intestinal nodule and the anal verge (p = 0.030) but was less tolerated than 3D-RWC-TVS (p < 0.001). (4) Conclusion: This was the first study comparing the performance of 3D-RWC-TVS and CTC in the diagnosis of rectosigmoid endometriosis. Both techniques allowed for the evaluation of the profile of the bowel lumen in a pseudoendoscopic fashion and had a similar performance for the diagnosis of rectosigmoid endometriosis, although CTC was more accurate in diagnosing and characterizing sigmoid nodules.
