Confocal Laser Endomicroscopy for Diagnosis and Histomorphologic Imaging of Brain Tumors In Vivo

Abstract: Early detection and evaluation of brain tumors during surgery is crucial for accurate resection. Currently cryosections during surgery are regularly performed. Confocal laser endomicroscopy (CLE) is a novel technique permitting in vivo histologic imaging with miniaturized endoscopic probes at excellent resolution. Aim of the current study was to evaluate CLE for in vivo diagnosis in different types and models of intracranial neoplasia. In vivo histomorphology of healthy brains and two different C6 glioma cell … Show more

“…Confocal fluorescence microscopy and photoacoustic imaging have also been explored for microscopic guidance in brain tumor surgery (30, 31). However, these methods rely upon the uptake of dyes, which are unevenly distributed within tumors (32, 33) and do not allow direct visualization of normal brain tissue that should be preserved.…”

“…Confocal microendoscopy combines the principles of confocal microscopy with fiber optics to visualize tissue at even higher resolutions in situ. Fluorescent agents are generally administered and video sequences then acquired of tissues using the technique, revealing cellular morphology and microarchitecture in a way previously only possible by cytological or histological analysis, respectively [34], [36], [37]. In an animal study using 30 mice implanted with GL261-luc cells, the use of indocyanine green with near-infrared laser confocal microendoscopy was able to effectively visualize the infiltrating tumor boundary and also identify satellite cancer cells within peritumoral tissue [37].…”

“…More recently, a total of 15 fresh specimens from patients undergoing surgical resection were analyzed ex vivo using confocal microendoscopy after being incubated with acruavine or fluorescein for 3-5 min at room temperature. Healthy brain, glioblastoma, meningioma, and metastases were all readily visualized using the technique [36]. The development and integration of these promising new optical imaging technologies into robotic systems may further improve the detection, characterization, and resection of tumors.…”

“…Pilot trials have evaluated rigid CLE for imaging of cervical intraepithelial neoplasia during colposcopy, using acriflavine [80]. Imaging to guide resection in neurosurgery is currently under evaluation [81][82][83].…”
