Congenital Aplasia of the Patella and the Distal Third of the Quadriceps Mechanism

Abstract: Case: Congenital aplasia or hypoplasia of the patella by itself is a rare condition; to our knowledge, it has been associated with aplasia of the quadriceps mechanism only 3 times in the literature. We describe a 5-year-old boy who had bilateral fixed flexion deformity of the knee as well as bilateral hypoplasia of the patella and quadriceps mechanism; he never had been able to stand upright and walk. We present the clinical and radiographic features, the surgical details, and the successful outcom… Show more

“…Furthermore, both muscles have a long tendinous portion and can easily be harvested using a tendon stripper. There’s no need to reroute the tendons to bring them in a more anterior position at the site of the natural distal insertion of the patellar tendon as it has been reported [ 5 , 6 ]. The proximal insertion over the top of the patella with the folding back of the tendons provide a strong proximal anchor and a thick new patellar tendon.…”

“…Congenital absence of patella is rare and is usually among the findings of the hereditary Nail Patella Syndrome, ischiopatellar dysplasia or orthopedic anomalies [ 36 , 37 ]. There were 9 subjects (2.4%) evaluated to have had ‘no patella’ with all 9 subjects’ ages ranging from 3 to 4 years old.…”

“…Congenital aplasia of the quadriceps muscle is a rare condition mentioned only a handful of times in the orthopedic literature. Most case reports describe an association with ipsilateral aplasia or dysplasia of the patella [1][2][3]. Ideally, treatment and surgical correction should start before adulthood.…”
