Congenital syphilis re-emergence in Winnipeg, Manitoba

Abstract: Background: Infectious syphilis rates have been increasing in Winnipeg, Manitoba among individuals during their childbearing years. Untreated or inadequately treated prenatal infection often results in congenital syphilis, with devastating consequences to fetal health and survival. The objective of this study was to review public health surveillance data regarding congenital syphilis incidence and birthing parent risk factors in Winnipeg from 2018 to 2020. Methods: Data extracted from a population-based surve… Show more

“…Similar observations were reported in neighboring countries, including Canada, with a 10- to 15-fold increase of congenital syphilis between 2016 and 2020 ( 16 ). Once again, the populations identified as most at risk were Native American and economically challenged patients: they face a lack of access to care, longer delays between diagnosis and treatment with many missed opportunities ( 16 , 34 ). Mexico was also affected by a 6-fold increase in the number of congenital syphilis cases between 2010 and 2019 ( 17 ).…”

“…Studies have established that late presentation for syphilis screening during pregnancy is a strong predictor of an infant with CS. 11,14,15 Social determinates of health including poverty and substance use are key factors in the current syphilis crisis. 11,14,15 There is a need for innovative methods to reach persons who experience barriers in accessing care.…”

“…This could be a result of many factors including increased incidence of syphilis in women and men who have sex with women (increasing the number of infectious individuals in a community), reduced effectiveness of partner services, changes in sexual behaviour or sexual networks, or even the effects of expanded electronic laboratory reporting by providers and laboratories (compared with telephone, fax or mailed case reports), which may have increased the capacity to report infections 10. Other countries and jurisdictions have similar screening and treatment protocols to Florida, and have found similar challenges in CS prevention including: lack of or inadequate prenatal care, maternal reinfections or mother’s testing positive after an initial negative syphilis test 19–21. Finally, these increases are not likely a result of an increase in number of live births in Florida as the number of live births only increased 1.5% from 2013–2014 (435 099 live births) to 2018–2019 (441 518) 22…”
