Background: The incidence of infective endocarditis (IE) among people who inject drugs (PWID) is rising worldwide. Multiple clinical guidelines differ on the management of this condition, and few guidelines comment on treatment for primary substance use disorder (SUD). A comprehensive comparison of these guidelines is lacking.Objectives: To perform a critical review identifying key differences in clinical guideline recommendations for treating IE among PWID, focusing on the inclusion of recommendations for SUD treatment and the presence of stigmatizing language.Eligibility Criteria: Recently published, English-language, society-developed clinical guidelines for the treatment of IE among PWID.

Infectious endocarditis (IE) represents an inflammation of the heart endothelium usually as a result of bacterial colonization after trauma. Splenic abscess is a rare and dangerous extracardiac manifestation of IE. An expert multidisciplinary collaboration is necessary for the diagnosis and treatment strategy of this high-risk and challenging disease. Valvuloplasty and splenectomy are the mainstay of radical treatment and offer excellent long-term results. We present two cases with infectious endocarditis, complicated with splenic abscesses, which were successfully operated in the Department of Cardiac Surgery.

The literature review presents an analysis of domestic and foreign publications, modern clinical guidelines for infective endocarditis. Based on the analysis of literature data, the main directions of examination of patients with infective endocarditis, including complicated forms of the disease, were determined. Approaches to the treatment and prevention of infective endocarditis are analyzed. Modern aspects of surgical treatment of this pathology are considered.
