Continuous-Flow Left Ventricular Assist Device Thrombosis: A Danger Foreseen is a Danger Avoided

Abstract: Left ventricular assist devices (LVAD) are an increasingly implemented therapeutic intervention for patients with end-stage heart failure. A growing body of evidence, however, has shown an elevated risk of device thrombosis, a major complication jeopardizing the patient’s post-implantation survival. To date, multiple causative factors for LVAD thrombosis have been identified, such as internal shear stress, device material, infection, and inadequate anticoagulation.Understanding the mechanisms leading to LVAD t… Show more

“…In a study in 13 patients with TAH, Ramirez et al found that bleeding complications occur generally between postoperative days 1 and 3 [12]. Patients with mechanical assist devices are prone to suffer from thrombotic events [13–16], device malfunctioning, wound infections, and bleeding complications.…”

“…Other complications related to LVAD infections can include hemorrhagic stroke and mycotic aneurysm. Patients with heart failure are already at risk of thrombosis, and increased infectious complications and coagulopathy associated with LVADs increases the risk of device thrombosis and stroke (reviewed in [54]). Aggarwal et al [55] studied the relationship between bacteremia and stroke in LVAD patients in a retrospective chart review study.…”

“…The modern treatment of heart failure is often associated with multiple surgical procedures and devices with potential complications (1,2). Due to the fact that HTX is gradually becoming a redo procedure, even more devices and strategies are being developed to facilitate faster organ offers (3,4).…”
