BRIEF SUMMARYCurrent Knowledge/Study Rationale: Botulinum toxin type A (BoNT-A) has been used for managing involuntary orofacial movements and secondary bruxism in patients with movement disorders, but its usefulness and objective effects on sleep bruxism (SB) have not been evaluated using objective measures. The aim of this study was to investigate the effects of intramuscular BoNT-A injection on orofacial motor events during sleep in patients with clinical diagnosis of SB. Study Impact: A single injection of BoNT-A into jaw-closing muscles is an effective strategy for controlling SB for at least a month. It reduces the intensity rather than the generation of the contraction in the jaw-closing muscles. Establishing management of SB with BoNT-A awaits further studies on larger samples over a longer follow-up period. Medicine, Vol. 10, No. 3, 2014 YJ Shim, MK Lee, T Kato et al equally effective in pain reduction when compared with oral splint. 17,18 The result of these studies and case reports were based on the subjective evaluation of SB, not on the objective assessment using electromyography (EMG) or polysomnography (PSG). A study using portable electromyographic (EMG) device showed that the counts of sleep related masticatory EMG bursts, detected by the pre-defined threshold, were significantly reduced after a single injection of BoNT-A in the masseter muscle. 19 However, it remains unclear how BoNT-A injection works specifically for sleep-related masticatory EMG activity, as a variety of jaw motor activities occur in association with transient arousal changes during sleep. 20 Video-polysomnography (vPSG) can be a useful tool allowing discrimination of the types of sleep-related masticatory EMG events. 2 In previous case reports and studies, some cases injected BoNT-A into the masseter muscle only, [13][14][15]17,18 and others into both the masseter and temporalis muscles. 10,11,15 The masseter and temporalis muscles are synergetic muscles and activated during teeth grinding and clenching; however, there have been no studies assessing the difference between the choices or the combination of the injected muscles for motor activity control of the jaw during sleep. 292 Journal of Clinical SleepTherefore, the aims of this study were to investigate the effects of intramuscular BoNT-A injection on jaw motor episodes using vPSG. In addition to the use of vPSG, we compared the effects of BoNT-A on jaw motor episodes between the injection sites (i.e., the masseter muscle injection only versus injection of both masseter and temporalis muscles). A preliminary study on the treatment of bruxism by biofeedback therapy

Objective: To evaluate the effect of wireless biofeedback therapy on bruxism. Methods: Fifteen bruxiers participated to be treated by this therapy. The abnormal movements of teeth during sleep were monitored by a maxillary splint with an electric resistance strain gauge. Meanwhile, the receiver device recorded these details and analyzed the data, including the value of the biting force, occurring time and duration. If the value of biting force or duration exceeds the threshold, a vibrating device like a watch style will alert the patient to relax the masticatory muscles and nervous system to stop the abnormal grinding or clenching. Data were recorded during 8 hours' sleep and analyzed after the 12 weeks' treatment and 24 weeks' treatment. Results: The average episodes of bruxism have declined from (10.60 ± 1.23) to (6.60 ± 0.75) after 12 weeks' treatment (p < 0.05), and the average duration of bruxism events was reduced from (13.2 ± 0.74) s to (6.50 ± 0.40) s (p < 0.05). The average episodes of bruxism have declined to (3.80 ± 0.64) after 24 weeks' treatment, and the average duration of bruxism events was reduced to (3.37 ± 0.34) s (p < 0.05). Conclusions: The results suggest that biofeedback therapy can be effective for bruxism treatment.

Ö ÖZ ZE ET T Toplumda yüksek oranda gözlemlenen temporomandibuler eklem (TME) rahatsızlıkları çene, boyun ve baş ağrıları, eklem sesleri gibi pek çok semptomla bireylerin yaşam kalitesini düşürmek-tedir. Bu rahatsızlıklar tedavi edilmediği takdirde ilerlemekte ve TME'de kalıcı hasara sebep olabilmektedir. TME rahatsızlıkları için pek çok tedavi yöntemi bildirilmiştir. Bu tedavi yöntemleri geri dönüşümlü (noninvaziv) ve geri dönüşümsüz (invaziv) tedaviler olmak üzere ikiye ayrılmıştır. Geri dönüşümlü tedaviler arasında bilgilendirme yöntemi, fizik tedavi yöntemleri, farmakolojik tedavi, psikiyatrik destek tedavisi ve oklüzal splint tedavileri yer alır iken; geri dönüşümsüz tedaviler oklüzal uyumlama ve cerrahi girişimleri içermektedir. TME rahatsızlıklarında geri dönüşümsüz tedavilerden önce daha konservatif yöntemler olan geri dönüşümlü tedaviler denenmesi temel prensiptir. Tavsiye edilen tedavi yöntemlerinin etkinliklerini araştırmak amacıyla pek çok çalışma yapılmıştır. Bununla birlikte, tedavi etmede ya da semptomları azaltmada hangi tedavi yönteminin ya da hangi splint tipinin daha etkin olduğuna dair kesin bir kanıt mevcut değildir. Etki mekanizmaları ve tedavi başarıları tartışmalı olmasına rağmen, TME rahatsızlıkları tedavisinde en çok kullanılan yöntem oklüzal splint tedavisidir. Diş hekimliğinde her biri farklı özel durumlar için olmak üzere birçok oklüzal splint kullanılmaktadır. Splint endikasyonları TME inflamasyonu, eklem içi dü-zensizlik, cerrahi sonrası tedavi, hipertonik yüz kasları, diş sıkma ve gıcırdatma olarak belirtilmektedir. These treatment methods are divided into two as non-invasive treatments and invasive treatments. Non-invasive treatments include informative methods, physical therapy, pharmacological treatment, psychiatric support therapy and occlusal splint therapies and invasive treatments include occlusal adjustment and surgical interventions. Non-invasive treatments, which are more conservative methods should be employed before invasive treatments for TMJ disorders.
