Influence of Core Competence on Voice Behavior of Clinical Nurses: A Multicenter Cross-Sectional Study

Background Voice behavior, referred to as a positive guarantee for organizational development, is influenced by several kinds of individual, collective and organizational features. However, the impact of individual competence on voice behavior is unclear. The aim of the present study was to investigate the status quo of core competence and voice behavior of clinical nurses and explore the impact of core competence on nurses’ voice behavior. Methods A multicenter cross-sectional survey. A total of 1717 nurses were recruited from nine tertiary and secondary hospitals between March and June 2019. An online questionnaire, including socio-demographic variables, employee voice behavior scale and competence inventory for registered nurses, was used to investigate prohibitive and promotive voice behavior and core competence of clinical nurses. Pearson correlation and hierarchical multiple regression were performed in the data analysis. Results The mean score for prohibitive and promotive voice behavior of nurses were 3.46 (SD 0.77) and 3.46 (SD 0.88), respectively. The mean score for core competence was 2.46 (SD 0.77). Critical thinking/research aptitude was the most important predictor for both prohibitive and promotive voice behavior (each p < 0.05), but its influence on promotive voice behavior was greater ( p < 0.05). Leadership was another significant predictor for prohibitive voice behavior ( p < 0.05). Legal/ethical practice, teaching-coaching, professional development and shift work were other predictors for promotive voice behavior (each p < 0.05). Conclusion Clinical nurses experience modest levels of prohibitive and promotive voice behavior and their core competence is moderate. Core competence, especially critical thinking/research aptitude, impacts significantly on voice behavior of clinical nurses. Cultivating nurses’ core competence could positively increase their voice behavior for organizational development.

Background: Nurses in public health centers, or known as public health nurses (PHNs), have duties in implementing promotive, preventive, curative, and rehabilitative efforts. In Indonesia, there is no basic reference to the competencies that should be performed by PHNs. The provision of health services in the community should cover two areas, namely inside and outside the building (Minister of Health’s Regulation No. 279 Year 2006); it causes nurses in the community to further hone their skills while in the field.Purpose: This study aimed to identify the competencies of the Indonesian PHNs from experts using the C/PHN competencies of the Quad Council Coalition through a Delphi method.Methods: This study was quantitative research with a Delphi method. We employed a purposive sampling technique to recruit the experts of public health nurses. The experts did a Delphi method to identify, analyze, and modify the C/PHN Competencies of the Quad Council Coalition into the Indonesian version of C/PHN Competencies with local cultures.Results: Results showed that from eight domains of the Quad Council Coalition C/PHN competencies, there were changes for priority of the PHN competencies in Indonesia. All priorities were classified into eight groups, namely: (1) Leadership Skills and Thinking Systems; (2) Community Dimensions of Practice; (3) Assessment Analytic; (4) Policy Development and Program Planning; (5) Communication Skills; (6) Financial Planning and Management and Planning; (7) Public Health Sciences Skills; and (8) Cultural Competency.Conclusion: The Quad Council Coalition of C/PHN competencies are appropriate with the Indonesian PHN competencies, although the priority is changed related of local wisdom as stated in the Minister of Health’s Regulation No. 279 Year 2006. Therefore, the Indonesian P/HN competencies should be developed to support the Indonesian health people through family approach.

Resumo Os direitos à saúde sexual e reprodutiva foram desenvolvidos recentemente advindos da luta pela cidadania e pelos Direitos Humanos. Embora haja um reconhecimento da necessidade de formação por competências nesta área, sua delimitação ainda não foi explorada no Brasil. Este trabalho teve como objetivo construir um referencial de competências transversais para a atenção em Saúde Sexual e Reprodutiva na Atenção Primária à Saúde a partir da visão de especialistas. Trata-se de um estudo exploratório-descritivo, de abordagem mista, através da técnica Delphi online, realizado com 41 especialistas em saúde Sexual e reprodutiva e direitos sexuais e reprodutivos. Foram realizadas três rodadas de Delphi. Das 36 competências resultantes da análise qualitativa, 32 alcançaram consenso, sendo classificadas em quatro domínios: ética e princípios profissionais; liderança e gestão; trabalho com a comunidade, saúde e educação, aconselhamento e avaliação; e, provisão do cuidado. Os resultados corroboram conteúdos de competências preconizadas na literatura internacional e apresentam inovações para a prática. Por serem transversais, estas podem subsidiar o planejamento de ações das equipes no âmbito da saúde sexual e reprodutiva.
