Coronary risk score for mineral bone disease in chronic non-diabetic hemodialysis patients: results from a prospective pilot study

Abstract: A noninvasive screening algorithm for early diagnosis of CAD in asymptomatic HD patients with altered MBD parameters is necessary. Applying MBD-coronary risk score might be an important step in the prevention of major coronary episodes by extending the indication for further investigations, early diagnosis and treatment management.

“…Mortality rate is considered around 15% 8 . Studies have shown that right colon ischemia or involvement of both right and left colon in patients with age over 50 years are associated with poor outcome 19 . Other risk factors for mortality are concomitant small bowel ischemia, chronic respiratory disease, especially chronic obstructive pulmonary disease 20,21 .…”

“…Studies on this pathological association showed that arterial occlusive disease is most frequent in patients with cardiovascular risk factors receiving steroid therapy (YARUR & al [39]). There are studies that also confirm the fact that in the active phase of the disease, and due to the corticosteroid therapy, the patients are at risk of developing bone fractures on the back of reduced bone mass, especially in patients with associated diabetes or renal disease, and they need a specific surgical management (NEAGU & al [40]; POIANA & al [41]; DAVID & al [42]). A large study, which assessed the risk of pulmonary embolism in patients with IBD, conducted on 55.496 subjects, observed an increased risk for pulmonary embolism in IBD, compared to the general population (BERNSTEIN & al [43]).…”

“…Recently, there is consensus that simple calcium intake does not increase coronary calcification [19]. However in the Multi-Ethnic Study of Atherosclerosis, despite high calcium intake was associated with lower risk of atherosclerosis in 5.448 patients without baseline cardiovascular disease, supplements use lead to a higher calcium coronary score [20,21]. An old small trial showed that oral calcium intake does not change serum osteoprotegerin and matrix Gla protein (as markers of tissue crystallization) or urine hydroxyproline excretion [22].…”
