Correlation between Patient-Reported Symptoms and Ankle-Brachial Index after Revascularization for Peripheral Arterial Disease

Abstract: Improvement in quality of life (QoL) is a primary treatment goal for patients with peripheral arterial disease (PAD). The current study aimed to quantify improvement in the health status of PAD patients following peripheral revascularization using the peripheral artery questionnaire (PAQ) and ankle-brachial index (ABI), and to evaluate possible correlation between the two methods. The PAQ and ABI were assessed in 149 symptomatic PAD patients before, and three months after peripheral revascularization. Mean PAQ… Show more

“…Staying physically active is an important goal for older adults as a determinant of the ability to sustain independent living. Because several lifestyle (eg, smoking cessation or supervised exercise) or medical (eg, cilostazol or revascularization) interventions have demonstrated improvement in physical function among patients with PAD,45, 46, 47 it seems important to identify the contribution of PAD to the reduction of physical components of QOL in older adults. It is important to keep in mind that reduced QOL was also observed in those with borderline low ABI (<1.00) in our study.…”

“…The transition probability to ‘moderate PAD’ was calculated from the probability of MALE, multiplied with the proportion of acute and chronic limb ischaemia from COMPASS; 22% of these patients transitioned to ‘moderate PAD. 15 The transition probability to ‘severe PAD’ was calculated from the number of major vascular amputations and the population at risk in COMPASS, assumed to be patients with a revascularisation history ( Tables 1 and 2 in Anand et al.). 1 No data were available to model PAD progression on clopidogrel, therefore probabilities were assumed equal to aspirin.…”

“…After 2 years of follow-up among 364 patients following an unscripted management plan for PAD, including a low rate of revascularization (18.9%), 36% were deemed treatment nonresponders with <5-point improvement in PAQ summary score. In another study, Je et al studied PAQ summary score change among 149 patients that underwent endovascular (n ¼ 100) and open surgical (n ¼ 49) revascularization for symptomatic PAD [23]. Among the study cohort, 22% failed to achieve an improvement in ABI !…”
