Correlation of Serum Albumin Level to Lung Ultrasound Score and Its Role as Predictors of Outcome in Acute Respiratory Distress Syndrome Patients: A Prospective Observational Study

Abstract: Background. There is ambiguity in the literature regarding hypoalbuminemia as a cause of extravascular lung water and acute respiratory distress syndrome (ARDS) outcomes. The aim of the study was to determine if low serum albumin on admission leads to lung deaeration and higher lung ultrasound score (LUSS) in ARDS patients. Patients and Methods. It was a prospective observational study in which 110 ARDS patients aged between 18 and 70 years were recruited. Serum albumin level and lung ultrasound score were ass… Show more

“…Overall, 13 cohort studies, including eight prospective studies [ 15 , 17 , 19 , 20 , 22 24 , 26 ] and five retrospective studies [ 14 , 16 , 18 , 21 , 25 ], were analyzed in this meta-analysis. The characteristics of the included studies are summarized in Table 1 .…”

“…Among the included patients, 343 patients (33.6%) died during hospitalization. The difference of LUS score at baseline between non-survivors and survivors were reported in 11 studies [ 14 – 17 , 19 21 , 23 – 26 ], and RR of in-hospital mortality between patients with high versus low LUS score were reported in six studies [ 18 , 20 , 22 , 23 , 25 , 26 ]. Median of LUS score was selected as the cutoff value in four studies [ 18 , 20 , 22 , 23 ], while the receiver operating characteristic (ROC) analysis derived cutoff value was used in the other two studies [ 25 , 26 ].…”

“…The common ones are fever, malnutrition in sepsis, gut losses, and losses from damage to the glomerular basement membrane and hence it cannot have sole control over the extravasation of fluids in the pulmonary interstitium. 26 , 29 Also, we obtained albumin values on day 1, and serial monitoring was not analyzed. In our study, we found that the cumulative fluid balance at 1 week was significantly lower amongst the EVLW+ group when compared to the other.…”

“…Besides, on the day of admission to ICU, serum albumin has nothing to do with the degree of pulmonary degassing described by LUSS in ARDS patients. Serum albumin level < 3.25 g/dL increases the chances of prolonging ICU stay (≥ 10 days), but it cannot predict the mortality rate 33 . This is also consistent with our conclusion, suggesting that ARDS may be the main risk of all-cause death on the 28th day.…”
