Cranial CT appearance of acute multiple sclerosis

Abstract: In acute multiple sclerosis, cranial computerized tomography (CT) may show periventricular and deep white matter contrast-enhanced lesions that are easier to see using 8 mm rather than 13 mm cranial CT sections. Follow-up studies show that the lesions evolve either to areas of density similar to the surrounding white matter or to low-density lesions. We presume they represent foci of active demyelination with extravasation of iodine through an altered blood-brain barrier. Enhanced cranial CT studies may be hel… Show more

“…1 Our case adds the unexpected find¬ ing of contrast enhancement occur¬ ring in a pathologically and symptomatically regressing lesion. Despite continued clinical improvement asso¬ ciated with a decrease of 20 mg/day of prednisone, the second CT scan find¬ ings of increasing volume and con¬ trast enhancement suggested a dicho¬ tomy in clinical manifestations and CT findings in this patient.…”

“…A 12-year-old boy became obtunded and developed bilateral facial weakness, dysarthria, ataxia, and a right hemiplegia over a 24-hour period. He had been given ampicillin for pharyngitis 1 week earlier. When the patient was at an outside hospital, his CSF contained 110 red blood cells and 39 mononuclear cells per cubic miliimeter and a protein level of 69 m d dl.…”

“…Serial scans were performed in cases 1, 2 and 3 and in each case some resolution of previous lesions was noted (see Table IV). These changes have been described in adults (Cala and Mastaglia 1976, Glydensted 1976, Aita e f al. 1978.…”
