Critical Care Management Following Lung Transplantation

Abstract: Postoperative critical care management for lung transplant recipients in the intensive care unit (ICU) has expanded in recent years due to its complexity and impact on clinical outcomes. The practical aspects of post-transplant critical care management, especially regarding ventilation and hemodynamic management during the early postoperative period in the ICU, are discussed in this brief review. Monitoring in the ICU provides information on the patient’s clinical status, diagnostic assessment of complications… Show more

“…Reduced systemic vascular resistance is the primary mechanism for persistent hypotension after correction of hypovolemia early post-lung transplant 79 . Beyond the effects of anesthesia and sedatives 80 , a short-lived distributive or vasoplegic shock picture can be observed, occasionally profound, mediated by a systemic inflammatory response likely related to surgical trauma and/or extracorporeal circuit use 79,81 . Norepinephrine (NE) is the first-line vasopressor as an alpha-and potent β 1 -adrenergic agonist, and is therefore effective in distributive and cardiogenic shock 82 .…”
