Introduction: Computed tomography guided fine needle aspiration cytology is a safe and well established technique for the diagnosis of lung and mediastinal lesions. This study was carried out in the department of pathology in the tertiary health care and teaching institute where radiology department is well established. The aim was to study the cytomorphology of the spectrum of the lung and mediastinal lesions and to correlate the cytological diagnosis with the radiological diagnosis. Material and methods: Computed tomography guided fine needle aspiration cytology was done in 90 pateints after taking written consent of the patient. Patients were explained the benefits and risks of the procedure. The results were analysed. Results: A total of 90 patients were included in the study. There were 81 patients of lung lesions and nine patients of mediastinal lesions. There were 63(70%) males and 27(30%) females. Adequate aspirate was obtained in 77 patients giving the adequacy rate of 85.55%. Adenocarcinoma was the most common type of lung malignancy and Non Hodgkins Lymphoma was the most common malignancy in the mediastinum. Inflammatory lesions were seen in 13 cases. Granulomatous lesions were found in six patients. One interesting case of hydatid with aspergillosis was diagnosed on cytology smears, which on radiology was diagnosed as neoplastic. Pneumothorax was seen in three patients. No patient required chest tube insertion. Conclusion: Computed tomography guided fine needle aspiration cytology is a safe and reliable method for the diagnosis of lung and mediastinal lesions. It can help in early diagnosis and initiation of the treatment avoiding major surgical procedures.

BACKGROUNDLung cancer is the major cause of cancer related deaths all over the world. Computed Tomography (CT) -Guided Fine Needle Aspiration Cytology (FNAC) of a lung mass is an effective modality to diagnose lung cancer. FNAC is a rapidly emerging diagnostic modality to assess the nature of radiologically demonstrated lung mass lesions. We wanted to study the usefulness of CT-Guided FNAC in the evaluation of cytopathological spectrum in lung lesions. METHODSA cross-sectional study was carried out in a total of 150 cases for a period of 5 years from November 2014 to November 2019 on cases of lung mass lesions with a strong clinical suspicion of pulmonary neoplasm. CT-Guided transthoracic FNAC was performed and cytology smears were stained with May-Grunwald-Giemsa (MGG) stain, conventional Papanicolaou (Pap) stain and Ziehl Neelsen (ZN) stain. Smears were examined under microscope and broadly categorized into unsatisfactory, tubercular, benign and malignant lesions. RESULTSA total of 150 cases of lung mass lesions were studied, of which 85 were males (78.4%). The mean age of the patients was 67 years. Maximum cases of benign lesions were chronic non-specific suppurative lesions (17.3%, 26 cases) followed by tuberculosis (3.3%, 05 cases). Regarding the malignant categories, adenocarcinoma (22.7%; 34 cases) was the most common malignancy followed by non-small cell carcinoma not otherwise specified (NOS) (14.7%; 22 cases), squamous cell carcinoma (8.0%, 12 cases) and small cell carcinoma (6.7%; 10 cases) respectively. CONCLUSIONSCT guided FNAC is a simple, less expensive, and reliable method for diagnosis of pulmonary lesions.

Background: Computed Tomography guided Fine needle aspiration cytology of lung mass is an effective modality to diagnose lung cancer. Its use has been extended in differentiating lung tumors into various cytomorphological types so that it becomes easier for further treatment. The study is aimed to study the morphological features of various types of lung lesions, correlate with histopathology wherever possible and evaluate the sensitivity, specificity, and accuracy of fine needle aspiration cytology procedure. Materials and Methods: It is an observational cross-sectional study conducted in the Department of Pathology of Gandaki Medical College and Teaching Hospital from April 2018 to December 2020 and 109 cases were included. Results: In the study, 109 cases were included, female were 61(56%) while males were 48(44%) with an M: F ratio is 1:1.2. The maximum number of cases was seen in the age >70 years (45.9%), mean age being 68.4years. Smoking was seen in 66(60.6%) cases. Squamous cell carcinoma (SCC) 32( 29.4%) was the most common cytological finding followed by adenocarcinoma 20(18.3%). Similarly, SCC was the most common histopathological finding which comprised 30( 53.5%) followed by adenocarcinoma 14(25%). Fine needle aspiration cytology and histopathological diagnosis are significantly associated with a p-value <0.001. The sensitivity, specificity, and accuracy of fine needle aspiration cytology were 76%,80%, and 76.6% respectively. Conclusions: Computes tomography-guided fine needle aspiration cytology is a relatively safe, inexpensive, and accurate procedure in the diagnosis of different lung lesions for early diagnosis and appropriate treatment.
