Curative resection of a primarily unresectable acinar cell carcinoma of the pancreas after chemotherapy

Abstract: Background: Acinar cell carcinoma (ACC) represents only 1-2% of pancreatic cancers and is a very rare malignancy. At the time of diagnosis only 50% of the tumors appear to be resectable. Reliable data for an effective adjuvant or neoadjuvant treatment are not available.

“…Considered the standard of care for pancreatic adenocarcinoma, palliative treatment with gemcitabine, either alone or as combination therapy, has shown benefit in some reports but disappointing in others17, 22, 24, 25. Some activity has been noted with 5-fluorouracil2, 19 and paclitaxel (present study23). Because of the small number of cases, it is difficult to draw definitive conclusions regarding the best approach to systemic therapy.…”

“…Others that have been used are gemcitabine, cisplatin, doxorubicin, irinotecan, oxaliplatin, docetaxel, capecitabine, 5-FU/leucovorin, erlotinib, sunitinib and sirolimus. If the acinar cell carcinoma is unresectable, patient should be treated by 5-FU chemotherapy either with a neoadjuvant or palliative intention [17,18]. Treated acinar cell carcinomas may show histologic changes including fibrosis and accumulation of histiocytes.…”

“…Therefore, effective chemotherapy is necessary to improve the clinical outcome. However, chemotherapy regimens have not been established due to the rarity of this disease [19,20]. A study carried out in the Memorial Sloan-Kettering Cancer Center of 18 ACC patients who received in-homogeneous chemotherapy regimens, containing Gemcitabine, 5-FU, Leucovorin, Mitomycin, Cisplatin, Cytarabine, Irinotecan, Caffeine and Doxorubicin, reported partial responses (PRs) and stable disease in some patients.…”
