Current Evidence in Gastrointestinal Surgery: Natural Orifice Translumenal Endoscopic Surgery (NOTES)

Abstract: Background Natural orifice translumenal endoscopic surgery (NOTES) is a technique that uses transvisceral access to perform surgical procedures entirely through a natural orifice. Despite the increasing awareness of NOTES, there remain obstacles to its technical feasibility and widespread acceptance. Furthermore, with the paucity of high-level supporting evidence, NOTES currently remains an experimental technique. Objective This article reviews the goals, applications, technical and practical challenges, and f… Show more

“…In October 2005, a scientific working group was also founded and named Natural Orifice Surgery Consortium for Assessment and Research (NOSCAR) to study the safety, efficacy and indications, as well as the ethical use of NOTES in humans; 5‐year progress on NOTES was published subsequently. Extensive investigation was carried out in animal models and cadavers exploring the safety and efficacy of NOTES, with a gradual transition to phase I clinical trials. Soon another milestone was realized, with the first transgastric cholecystectomy in a human performed by Jacques Marescaux.…”

“…NOTES benefits include improved cosmesis, and improved postoperative pain. 1 According to reported statistics, 2 916 patients received NOTES between 2007 and 2011 worldwide, in which 79% were transvaginal. The natural orifice operative approach remains low in case volume compared with laparoscopic interventions and tremendous progress is needed to make this surgery practical in clinical application.…”
