Cutaneous leiomyosarcoma with myxoid alteration arising in a setting of multiple cutaneous smooth muscle neoplasms

Abstract: Myxoid leiomyosarcomas have exclusively been reported in the uterus and gastrointestinal tract, but not in the skin. Here, we describe a case of cutaneous leiomyosarcoma with myxoid alteration that developed in the excision scar of a piloleiomyoma and was accompanied by a cutaneous angioleiomyoma in a different location.

“…It is possible that spillage of the gelatinous matrix at the time of surgery contributes to the phenomenon of local recurrence. Pure myxoid LMS are exceedingly rare in the skin, 25 while they are most common in the uterus and deep soft tissues, where they have been associated with aggressive clinical outcomes 31,37 …”

“…Thus, these tumours have been poorly characterized in terms of both pathological features and clinical outcome. To date, LMS of the skin has been analysed only in a few studies of any significant size 1–7 and in numerous smaller investigations 8–26 . In most of these studies, the specific pathological features and clinical outcome of cutaneous LMS are difficult to establish because true cutaneous LMS have not been analysed separately from subcutaneous LMS.…”

“…The natural history of cutaneous leiomyosarcoma is not clearly understood 27 . Factors proposed to predispose to this form of leiomyosarcoma include preexisting leiomyomas of long duration; 27 antecedent of physical trauma, including previous surgical excision; 27,28 and exposure to radiation 21,29 .…”

“…On palpation these tumour do not invade to dermis and are fixed to skin. 5 In more than 80% of the multiplicity is common 6 , and most commonly affects trunk, extremity and face. Asymmetrical pattern is the most common.…”
