Expressions of activation markers have been described on the surface of T cells in the blood and the lung in both health and disease. We have studied the distribution of activation markers on human lung T cells and have found that only certain populations exist. Importantly, the presence or absence of some markers appears to predict those of others, in particular cells which express CD103 also express CD49a and CD69, whereas cells which do not express CD69 also do not express CD49a or CD103.In view of the paucity of activation marker expression in the peripheral blood, we have hypothesised that these CD69+, CD49a+, and CD103+ (triple positive) cells are retained in the lung, possess effector function (IFNγ secretion) and express particular chemokine receptors which allow them to be maintained in this environment.We have found that the ability of the triple negative cells to secrete IFNγ is significantly less than the triple positive cells, suggesting that the expression of activation markers can highlight a highly specialised effector cell. We have studied the expression of 14 chemokine receptors and have found that the most striking difference between the triple negative cells and the triple positive cells is the expression of CXCR6 with 12.8±9.8% of triple negative cells expressing CXCR6 compared to 89.5 ±5.5% of triple positive cells.We propose therefore that CXCR6 may play an important role in the retention of T cells within the lung.

Aim: Diagnostic values of soluble SR-PSOX/CXCL16 (sSR-PSOX/CXCL16), a receptor for atherogenic oxidized LDL and a membrane-anchored chemokine for CXCR6-positive lymphocytes, for acute coronary syndrome (ACS) were evaluated. Methods: We examined 106 patients undergoing coronary angiography (CAG); 17 patients with ACS and 89 patients without ACS (non-ACS) including stable angina. Circulating sSR-SPOX/CXCL16 was measured in peripheral venous blood by sandwich ELISA. Results: Age, gender, prevalence of diabetes or hypertension, and serum lipid profiles were not significantly different between ACS and non-ACS. Presence or absence of risk factors, such as diabetes, smoking and hypertension, did not significantly affect circulating sSR-PSOX/CXCL16 levels. Circulating sSR-PSOX/CXCL16 levels were significantly lower in ACS than non-ACS (median: 3.05 versus 3.36 ng/mL, p 0.02). Lipid profiles, high-sensitivity C-reactive protein (hs-CRP), cardiac troponin T (TnT), and soluble LOX-1 (sLOX-1) showed no significant correlation with sSR-PSOX/ CXCL16. Receiver-operating characteristic (ROC) curves for ACS detection indicate higher sensitivity and specificity for sSR-PSOX/CXCL16 than hs-CRP. In the TnT-negative and sLOX-1-negative subpopulation, sSR-PSOX/CXCL16 showed similar sensitivity and specificity for ACS; however, hs-CRP showed less sensitivity and specificity for ACS when compared with the whole population. There is no reliable way to identify the high-risk patients with intermediate coronary artery lesions (diameter stenosis 20%-70%) in early stage. Soluble CXC chemokine ligand 16 (CXCL16) is a newly discovered chemokine that can mediate inflammatory responses. It is released by proteolytic cleavage of its membrane-bound form, named scavenger receptor for phosphatidylserine and oxidized lipoprotein (SR-PSOX) that can promote the uptake of oxidized low-density lipoprotein cholesterol by macrophages. We have hypothesized that CXCL16 is an indicator of the prognosis of intermediate coronary artery lesions, and thus assessed the association between plasma CXCL16 concentrations and the 2-year prognosis in 616 patients with intermediate coronary artery lesions. The primary endpoint was a composite of all-cause death, non-fatal myocardial infarction, revascularization and angina pectoris requiring re-hospitalization. During the median follow-up time of 24 months, 69 events occurred. The plasma concentrations of CXCL16 (median 7712.88 pg/ml vs. 6792.43 pg/ml, P = 0.014) and high-sensitivity C-reactive protein (hs-CRP) (median 2.82 mg/L vs. 1.68 mg/L, P < 0.001) were higher in patients with events than patients without events. Cox hazard proportion analysis showed patients in upper CXCL16 quartile were more likely to suffer from adverse outcome than patients in lower quartile (RR = 1.271, P = 0.029, 95% CI: 1.025-1.577) after adjusting for sex, age, smoking, hypertension, diabetes, fat, dyslipidemia, hs-CRP, and medication use. In conclusion, plasma level of CXCL16 is an independent predictor of the prognosis of the patients with intermediate coronary lesions. Elevated plasma CXCL16 is associated with higher risk for these patients.
