Dedifferentiated Paratesticular Liposarcoma with Osseous Metaplasia

Abstract: Paratesticular liposarcoma is a rare tumour of the genitourinary track but the most common of all sarcomas in adults. The dedifferentiated variation occurs only in 10% of liposarcoma cases. The typical clinical presentation is similar to an inguinal hernia or a benign lipoma. We present the case of a dedifferentiated paratesticular liposarcoma with osseous metaplasia of the spermatic cord, in a male presented with acute scrotum.

“…According to the literature, spermatic cord DDLs originate in the distal and middle portions of the cord. DDLs with ossification located in the retroperitoneum are rarely reported, and there are no reports of spermatic cord DDL with macroscopic ossification, similar to that in the present case. Due to its rarity, the clinical role of ossification within DDL remains unknown.…”

“…Liposarcomas are located most commonly in the lower extremities or the retroperitoneum, but spermatic cord liposarcoma is rare. Few cases of spermatic cord DDL have been published in the literature, and there are no reports on spermatic cord DDL with macroscopic ossification . The available literature states that the preferred treatment for spermatic cord liposarcoma is surgical resection using orchiectomy with high cord ligation for tumors with negative surgical margins .…”

“…Leiomyosarcoma (32%), rhabdomyosarcoma (24%) and liposarcoma (20%) are the most common sarcoma subtypes (7). Liposarcomas are histologically well divided into four groups as differential, dedifferentiated, myxoid round-cell and pleomorphic liposarcoma (8). Approximately 50% of paratesticular liposarcoma cases are well differentiated liposarcoma.…”

“…In addition, high grade pleomorphic sarcoma, pleomorphic leiomyosarcoma, pleomorphic rhabdomyosarcoma, pleomorphic malignant peripheral nerve sheath tumor and malignant fibrous histiocytoma should be evaluated separately (8,9). Histologically pleomorphic liposarcoma is diagnosed by demonstrating pleomorphic lipoblasts in hematoxylin eosin (HE) sections.…”

“…Makroskopik olarak lobüle, iyi sınırlı, normal yağ dokusu görünümünde, kimi zaman 30 cm çapa ulaşan kitle izlenir. Yerleşim yeri çoğunlukla spermatik kord ve epididimdir (4,19). Histopatolojik olarak en sık görülen alt tip iyi diferansiye/dediferansiye liposarkomdur (Resim 4) (4,17,18,19,20).…”
