Deep Venous Thrombosis and Pulmonary Embolism are Uncommon in East Asian Patients after Total Hip Arthroplasty

Abstract: Background In Western countries, deep vein thrombosis (DVT) and pulmonary embolism (PE), are relatively common after THA and many surgeons recommend routine pharmacologic thromboprophylaxis. There is some suggestion in the literature that the incidences of DVT and PE may be lower in East Asian patients. Therefore, it would be important to establish the incidences in a large number of East Asian patients who did not receive pharmacologic thromboprophylaxis.Purpose We therefore determined the incidence of DVT an… Show more

“…27,28 It has been believed that the incidence of DVT after THA and TKA is lower in Asian populations as compared to Western populations. [29][30][31] However, recent multi-centric prospective studies conducted in Asia have shown the occurrence of VTE after major orthopedic surgeries, including THA and TKA, in comparison with Western populations. [32][33][34][35][36] Incidences of clinically relevant symptomatic VTE after hip and knee arthroplasty have also been reported in East Asian populations.…”

“…[32][33][34][35][36] Incidences of clinically relevant symptomatic VTE after hip and knee arthroplasty have also been reported in East Asian populations. 30,31 In a systemic review and meta-analysis, Kanchanabat et al 5 examined the incidence of VTE after orthopedic surgery in Asian patients without thromboprophylaxis and found very low incidence of proximal and symptomatic DVT as compared to Western populations, and no patient died of DVT. 35 Interracial differences, genetic factors, and a Western lifestyle may affect the occurrence of DVT.…”

“…In addition, we did not use routine chemical thromboprophylaxis, which has been considered as a risk of PJI28). Although several studies recommended the use of chemical thromboprophylaxis after THA, we did not do so as venous thromboembolism is uncommon in East Asian patients after THA2930).…”

“…However, in this study, hospital stay was prolonged in VTE patients (26.3±17.1 vs. 37.7±15.4, p =0.012). After surgery, delayed mobilization, a well-known risk factor for VTE,23-26 might be the reason for the pronged hospital stay and increased risk of VTE occurrence in these patients. In addition, titration of oral warfarin might be necessary in patients with symptomatic VTE before discharge.…”
